Objectives:Based on the results of prospective cohort studies in existing relevant fields, the literature was screened according to the type of prevention, study design, geographic region,follow-up time, and method quality.Meta-analysis was performed to determine obstructive sleep after excluding cardiovascular disease effects.Obstructive sleep apnea(OSA)has an impact on the risk of stroke in different populations and predicts the degree of impact;provides a theoretical basis for active treatment of OSA and prevention of its complications. Methods:This article adopts Chinese "obstructive sleep apnea" and "stroke",English "Sleep Apnea,Obstructive","Stroke" as keywords, using the theme + free word model,in Pubmed,Cochrane,Embase,Web of Science The China Knowledge Network(CNKI)full-text database and the China Biomedical Literature Database(CBM)conducted a comprehensive, prospective cohort study of “obstructive sleep apnea on the risk of stroke ” before February 28,2019.Systematic retrieval, supplemented by manual retrieval. The extracted literature was screened using rigorous inclusion/exclusion criteria. The final screened literature was evaluated for quality by the Newcastle-Ottawa Scale(NOS),data was compiled for quality-qualified literature, and excluded using STATA14.0 software.Meta-analysis of the single-group rate of OSA patients with cardiovascular disease on the incidence of stroke, with the risk ratio and 95% confidence interval as the evaluation criteria, using the random effects model to plot the forest map and funnel plot, and the heterogeneity rate Perform subgroup analysis and sensitivity analysis,as well as publish bias analysis. Results:In this study,a total of 15 articles were included, including 48,892 OSA patients without cardiovascular and cerebrovascular diseases. The risk of stroke in these OSA patients was analyzed. We obtained the risk of stroke in people with OSA [HR value and 95 The % confidence interval was 0.05(0.04,0.060)],and was statistically significant(P < 0.05),and the Eurasian population with OSA was found to be more correlated with the risk of stroke in the American population through subgroup analysis, and The difference was statistically significant(P<0.05);the difference between large sample(total number of samples >1000)and small sample(total number of samples ?1000)in OSA patients was statistically significant(P<0.05);OSA There was a statistically significant difference in the risk of stroke between the course of disease >5 years and ? 5 years(P<0.05).Whether the risk of stroke in patients with OSA was diagnosed by PSG alone was statistically significant(P<0.05)..Conclusion:After excluding the effects of cardiovascular disease,OSA can indeed increase the risk of stroke. The Eurasian population with OSA is about 1.5 times more likely to have a stroke risk than the American population;and the longer the duration of OSA,the stroke The greater the risk of increased risk.
