| With the continuous improvement of people’s living standards,obesity has been listed by the World Health Organization as one of the diseases that threaten global human health.Obese people often have metabolic diseases such as type 2 diabetes and hypertension.Insulin resistance has attracted widespread attention as a basis for the pathogenesis of various metabolic diseases,and it is closely related to diseases such as obesity and type 2 diabetes.An important cause of insulin resistance is an increase in the level of inflammation in the body,which promotes the formation of insulin resistance by interfering with the insulin signaling system and causing endothelial dysfunction.Chronic inflammation in obese people is usually elevated by systemic inflammatory factors(IL-6,TNF-α)2-3 times is characteristic.Exercise is an effective way to improve insulin resistance,but it is often difficult to adhere to.Today,people are increasingly nervous about work and study.The lack of exercise time is the main reason why people refuse to exercise.How to better integrate sports into public life has become a key issue that needs to be solved urgently.Studies have pointed out that the use of stair walk instead of elevator can effectively improve the aerobic capacity,waist circumference,fat mass,diastolic blood pressure and insulin sensitivity of sedentary people,which provides a new idea for people’s daily exercise,However,the separately effects of the descending and ascending stair training are not clear and further research is needed.Objective: To compare the effects of 12-week upstairs and downstairs training on insulin sensitivity,inflammatory factors,balance ability and muscle strength in obese young adults.Methods: Thirty-two subjects were randomized into two groups: the upper stair group(n=16)and the lower stair group(n=16),and both groups completed a 12-week(36 total)exercise intervention.Use the public staircase to exercise.In the first week,each training will repeat 5 groups of descending and ascending stair walking,and then increase 2 groups per week.To 12 weeks,each training will need to repeat up/down 27 groups.The single group movement needs 1 step/s speed up/down 96 steps.Before and after the experimental intervention,the FUR,the isometric muscle strength tester and the isokinetic strength tester were used to test the balance ability,the isometric muscle strength and the isokinetic muscle strength,and the subjects were tested for fasting insulin in the morning fasting blood.INS,GLU,OGTT,HOMA-IR,IL-6 and TNF-α.During the intervention,the average daily energy intake of the subjects and the amount of physical activity per week were consistent with those before the intervention.Results: After exercise intervention,the blood glucose in the lower floor group was lower than that before the intervention(P<0.05),but there was no statistical difference between the upper floor group and the intervention group(P>0.05).There was no statistical difference between the two groups.Learning differences(P>0.05).After 12 weeks of training,the insulin in the lower floor group decreased compared with the intervention,the statistical difference was very significant(P<0.01),the insulin in the upper floor group also decreased before the intervention,the statistical difference was significant(P<0.05),the insulin in the lower floor group The reduction was greater than that of the upper floor group(the interaction effect was very significant,P < 0.01).After exercise intervention,HOMA-IR in the lower floor group decreased significantly(P<0.01),and there was no significant change in the upper floor group(P>0.05).The reduction of HOMA-IR in the lower floor group was greater than that in the upper floor group(the interaction effect was very significant,P < 0.01).After exercise intervention,there was no significant change in OGTT between the two groups(P>0.05),and there was no significant difference between the two groups(P>0.05).After exercise intervention,the TNF-α levels in the two groups were lower than before the intervention,and the statistical difference was very significant(P<0.01).The decrease of TNF-α in the lower floor group was greater than that in the upper floor group(the interaction effect was very significant,P<0.01).After 12 weeks of training,the IL-6 levels in the two groups were lower than those before the intervention,and the statistical difference was very significant(P<0.01).There was no significant difference between the two groups(P>0.05).After exercise intervention,the balance ability of the closed-eye stable plane in the lower floor group was significantly increased(P<0.05).There was no significant difference between the upper-floor group and the intervention group(P>0.05).There was no statistical difference between the two groups.P>0.05).After exercise intervention,the muscle strength of the legs in the lower floor group was significantly increased(P<0.01),and the muscle strength of the upper leg group was not significantly different from that before the intervention(P>0.05).There was no statistical difference between the small amplitudes(P>0.05).After 12 weeks of training,the muscle strength of the left leg extensor in the lower floor group was significantly higher than that before the intervention(P<0.01).There was no significant difference in the muscle strength of the left leg extensor in the upper floor group compared with the intervention.The increase of the extensor muscle strength of the left leg of the lower floor group was greater than that of the upper floor group(interaction effect was significant,P<0.05).The muscle strength of the right leg extensor muscle,left leg flexor muscle and right leg flexor muscle were significantly increased in the two groups(P<0.05),and there was no significant difference between the two groups(P>0.05).Conclusion:(1)12 weeks of descending stair training can improve the insulin sensitivity of obese young people,but the ascending stairs training has no such effect,improvement of the descending stair training is more obvious.The mechanism by which downstairs training improves insulin sensitivity may be related to a decrease in TNF-α levels.(2)12 weeks of descending stair training can increase the balance ability of obese youth,and no effect on the ascending stair training.The reason for improving the balance ability after the descending stair training may be related to the increase of skeletal muscle strength. |
