Clinical Effect Of Vertical Incision Inner Pedicle On Breast Hypertrophy

Background Female Breast hypertrophy is caused by various causes,including spontaneous onset without obvious etiology,endocrine hormone stimulation and drug induction.There are also some rare factors,such as hypercalcemia,immune factors,epidermal growth factor,etc.Bring about mammary gland body,adipose reach connective tissue excessive development hyperplasia,cause mammary bulk to increase.The breast of excessive hypertrophy often brings double anguish of body and mind to the woman,affect patient life and working quality badly.At present,the treatment of breast hypertrophy is mainly reported by surgical methods at home and abroad.But its skill is various,have advantage and disadvantage each.Objective To summarize the clinical experience of vertical incision combined with internal pedicle method in the treatment of breast hypertrophy,and to provide new ideas for the diagnosis and treatment of breast hypertrophy in the future.Methods Through literature review and combined with clinical practice,all the 5 patients with moderate and severe breast hypertrophy in our department were treated with vertical incision combined with internal pedicle method,and the dermal gland pedicle with nipple and areola was designed preoperatively.Inaddition to the scope of new areola,the rest areas of the dermal gland pedicle were completely exfoliated and only the dermis was retained.According to the preoperative drawing line,the breast skin was cut vertically from the bottom to the top from the lower part of the breast,and the excess glandular tissue was removed in a wedge shape.The remaining glandular tissue was shaped and sutured layer by layer.Postoperative outpatient service and telephone follow-up were conducted for 6-12 months to evaluate the appearance,size and incision scar of the breast,and to observe whether there was nipple necrosis and hypoesthesia.Results The breast volume of 5 patients(10 sides in total)decreased obviously after operation.The incisions healed in stage I in 4 cases.Follow-up6-12 months showed no obvious scar hyperplasia,no recurrence of breast hyperplasia,bilateral nipple pressure sensation was basically consistent with that before surgery,long-term breast shape was beautiful and stable,and no secondary breast ptosis occurred.After surgery,1 patient had unilateral nipples and areola blackness accompanied by hypoesthesia,and after treatment,unilateral nipples were absent,leading to scar healing.Conclusion The causes of breast hypertrophy are complex and the clinical treatment methods are diverse.Domestic and foreign scholars have studied and reported on the advantages and disadvantages of different surgical methods,and found that none of the surgical methods can fully adapt to each patient at present,and there is still a lack of a relatively complete and systematic standard treatment program internationally.Through literature review and combined with clinical practice,we found that vertical incision combined with internal pedicle method had a better treatment effect for patients with moderate breast hypertrophy,while the postoperative effect was not good for patients withsevere breast hypertrophy.Therefore,in clinical practice,it is still necessary to select the optimal treatment method according to the specific situation of patients.
