| Objective Diffuse large B cell lymphoma(DLBCL)is a clinical and genetic heterogeneous disease.The standard first-line treatment is R-CHOP chemotherapy,because of the differences between individuals,its efficacy is not ideal.With the development of epigenetics,the role of histone modification in DLBCL was found to be more complex and diverse,and the genomics technique also identified a variety of mutations among histone methyltransferases.Recently,the sequence of exon and transcriptome of DLBCL gene revealed that the mutation rate of histone methyltransferase KMT2 D mutation ranks first among all mutations and the mutation rate of EZH2 was also very high.So,this study focused on the expression of KMT2 D and EZH2 in DLBCL,and the relationship between different expression levels and clinical pathological features,in order to provide possible predictors for the progression and prognosis of DLBCL,and provide possible new targets for the treatment of patients.Methods 108 cases of paraffin-embedded tissue specimens with DLBCL diagnosed in the Fourth People’s Hospital of Yunnan Province from 2008 to 2018 were collected as experimental group.Ten specimens of paraffin-embedded tissue of chronic reactive tonsillitis were selected as control group.The expression of histone methyltransferases KMT2 D and EZH2 in DLBCL paraffin-embedded tissues was examined by immunohistochemistry(Envision TM two-step method)and clinical pathological correlation analysis was carried out.Results In DLBCL,the expression level of KMT2 D was negatively correlated with clinical stage(P<0.05,r=-0.192),negatively correlated with Ki-67 expression in patients over 60 years old(P<0.05,r=-0.610),and positively correlated with prognosis(P<0.05,r=0.309);the expression level of KMT2 D was not associated with gender,age,primary site,immunological type,IPI risk score or BCL-2 expression(P>0.05).The expression level of EZH2 was positively associated with the expression level of Ki-67(P<0.05,r=0.230),but was not correlated with gender,age,primary site,immunological classification,clinical stage,IPI risk score,BCL-2 expression or prognosis(P>0.05).There was no correlation between KMT2 D and EZH2 expression levels(P>0.05).Conclusion The expression level of KMT2 D in DLBCL was negatively correlated with clinical stage,positively correlated with the prognosis of patients and was negatively correlated with the expression level of Ki-67 in elderly patients over 60 years old.These results may indicate that when KMT2 D is low expression,the patient is in the advanced stage of the disease and the tumor cells are actively proliferating in elderly patients,and also indicate KMT2 D may be an indicator of prognosis.EZH2 is positively correlated with the expression level of Ki-67,which may indicate that EZH2 is associated with tumor cell proliferation and the degree of malignancy. |
