Objective: To study the efficacy and safety of vestibular migraine in the treatment of vestibular migraine,we designed the investigation.The method combined physical effects with meridian acupoints,which was expected to be non-invasive,easy to operate,safe and effective for the treatment of vestibular migraine.Then we investigated the relationship between TCM syndrome types and videonystsgmography results by retrospectively analyzing the characteristic of videonystsgmography in the patients with vestibular migraine.Methods: According to the inclusion and exclusion criteria,50 patients diagnosed of vestibular migraine from the Department of Neurology,Wuhan Hospital of Integrated Traditional Chinese and Western Medicine,were randomly divided into the treatment group and the control group by each 25 patients.The control group was treated with oral flunarizine 5mg per night in strict accordance with the guidelines for drug use and drug instructions at home and abroad.The experimental group was treated with percutaneous acupoint electrical stimulation on the basis of the control group,and the bilateral wind pool points and days were selected.Stimulation of the acupoints and Fenglong acupoints,the sparse wave stimulation was maintained for 20 minutes,once in the morning and evening,after one week of continuous treatment,the clinical total efficacy,the vertigo disorder scale score,the TCM syndrome score,the visual simulation score,and the evaluation of the percutaneous acupoints were evaluated.The early efficacy of electrical stimulation in the treatment of vestibular migraine and the treatment of vestibular migraine with TCM syndromes are more effective.Through retrospective analysis of 50 cases of vestibular migraine patients' nystagmus view results,the distribution characteristics of nystagmus view results of vestibular migraine patients and their relationship with different TCM syndromes were discussed.Results:1.A total of 50 patients(including 13 males and 37 females)were included in the study.The gender,age,general past history,pre-treatment,DHI scale scores,TCM syndrome scores,VAS scores and TCM syndromes of the two groups before the study had no significant difference(P>0.05).2.There were significant differences in the scores of DHI scale scores,TCM syndrome scores and VAS scores before and after treatment in both groups(P<0.05).And the treatment group was superior to the control group.The difference in scores was statistically significant(P < 0.05).3.After one week of treatment,the total effective rate of the treatment group was 84.00%,and the control group was 56.00%.The treatment group was better than the control group,and the difference was statistically significant(P<0.05).4.The scores of DHI scale before and after treatment in patients with turbid middle-resistance vertigo were statistically significant(P<0.05),and the differences between TCMSSS and VAS scores were not statistically significant(P>0.05).There were significant differences in DHI and VAS scores before and after treatment with Fengyang disturbance(P<0.05).There was no significant difference in TCMSSS scores(P>0.05),and the score of DHI on Fengyang was higher than that.The turbidity resistance type is more obvious.There were no significant differences in DHI,TCMSSS and VAS scores between the patients with qi and blood deficiency and liver and kidney yin deficiency before and after treatment(P>0.05).The number of cases of blood stasis was too small to be statistically analyzed.5.A total of 33 patients with VM agreed to the videonystsgmography examine,which indicated 23 cases of lesions around the vestibular.The proportion was 69.70%,suggesting that 4 cases of vestibular central lesions alone,the proportion was 12.12%,6 cases coexisted,The ratio is 18.18%.74.36% suggested lesions around the vestibule,and 25.64% suggested vestibular central lesions.6.The smooth tracking test and the gaze test were normal.Most of the saccade test,visual test,spontaneous test and displacement test were normal,and the proportions were 90.91%,96.97%,90.91%,and 85.71%,respectively.The abnormal proportion of shaking head test and temperature test was slightly higher,39.40% and 46.67%,respectively.There was no significant difference between the two(P>0.05).7.It is suggested that the vestibular central lesions are mainly liver-kidney yin deficiency type,the proportion is 50.0%,followed by turbid middle resistance type and qi and blood deficiency type,the proportion is 25.0%.It is suggested that the lesions around the vestibule alone are mainly wind-yang disturbance type and turbid medium resistance type,the proportions are 56.52% and26.08%,respectively.It is suggested that the coexistence of the two is mainly for the wind-yang disturbance type and the liver-kidney yin deficiency type,the proportion is 33.3%.There was a statistically significant difference between the groups(P<0.05).8.There was no significant correlation between turbidity,qi and yang,qi and blood deficiency,phlegm and blood stasis and VNG distribution(P>0.05),and liver and kidney yin deficiency may affect the distribution of VNG results.(P<0.05).Conclusion: Transcutaneous electrical acupoint stimulation can significantly improve the severity of vertigo in patients with vestibular migraine,reduce the score of vertigo disorder scale and TCM symptom score and visual analogue score.It has significant clinical effects,including Fengyang upper disturbance type and the turbid middle resistance type is more effective.Moreover,the treatment method is non-invasive,and fully combines the interaction of electrical stimulation and acupuncture points.Compared with acupuncture and acupoint injection therapy,there is no possibility of vignetting by percutaneous acupoint electrical stimulation,and the operation is simple and safe.It is also easy for clinical patients to accept.The results of videonystsgmography in most patients with vestibular migraine suggest peripheral lesions,mainly shaking head test and temperature test abnormality.The smooth tracking test,visual test,gaze test and fixation inhibition are mostly normal.There was no significant correlation between the turbid middle resistance type,the Fengyang upper disturbance type,the qi and blood deficiency type and the blood stasis type and the VNG distribution.The liver and kidney yin deficiency type may affect the distribution of VNG results.
