[purpose]The aim of this study was to investigate the relationship between bendopnea and heart failure,to analysis the influencing factors of bendopnea,and to deduce the relationship between bendopnea and various indexes of heart failure by means of an investigative study;A longitudinal study was conducted to explore the effect of bendopnea on the quality of life and prognosis of patients with heart failure,to explore the predictive effect of bendopnea on assessing symptoms of heart failure,and to provide a new basis for the assessment of symptoms of heart failure.[methods]The study is divided into two parts.The first part is a cross-sectional survey.A total of 194 patients in our hospital,Department of Cardiology with heart failure were selected from March to September,2018.Survey methods include questionnaires,on-site evaluation,and electronic medical records.The contents of the questionnaire included general demographic data,disease status,and Minnesota heart failure quality of life questionnaire Chinese version.The on-site evaluation included physical indexes,symptoms and signs of the disease,6min walking test,and so on.Electronic medical records include Laboratory test results,electrocardiogram and ultrasound echocardiography.The second part uses longitudinal study,patients with heart failure who were admitted to our department of cardiology from March to September 2018 and followed up in our hospital's heart failure specialist clinic within the next 6 months were finally included in 60 patients with bendopnea group and 60 patients with no bendopnea group.The methods of investigation were 1 month,3 months,6 months after discharge by telephone and out-patient follow-up.The follow-up included Minnesota heart failure quality of life questionnaire Chinese version,status of life,re-admission rate,cardiovascular adverse events,BNP,ultrasound echocardiography,etc.Statistical method use SPSS23.0 software statistical data,and p<0.05 considered to be statistically significant.[results]In the first part,194 patients with heart failure were included,73 cases with bendopnea,accounting for 37.6%of the total survey,and 121 cases without bendopnea,accounting for 62.4%of the total survey.There is no statistical difference in the general demographic characteristics between the two groups.The results of univariate analysis show that there were statistical differences in the history of hospitalization,the number of hospitalizations due to heart failure in the last year,the number of days in the hospital,with hypertension,PCI treatment history,the New York Heart function Classification,and 6-minute walking test of two groups.The results of univariate analysis in physical index show that the body weight,BMI,waist circumference,hip circumference and waist hip ratio in the patients with bendopnea are higher than those in the patients without bendopnea,and show significant differences.The results of univariate analysis in sings and symptoms show that higher diastolic pressure in patients with bendopnea,and the incidence of end-sitting breathing,nocturnal paroxysmal dyspnea,edema and lung rales are higher than that of non-bendopnea group with statistical differences.The results of univariate analysis of laboratory parameters show that BNP and LDL in patients with bendopnea were higher than those without bendopnea,and there is a significant difference between the two groups.The results of univariate analysis of electrocardiogram and ultrasound echocardiography show that heart rate,LVDd,AoD and pulmonary artery systolic pressure in patients with bendopnea are higher than those in patients without bendopnea,and LVEF is to the contrary and there are statistical differences.The results of multivariate logistic regression analysis show that the hospital stay,hypertension,PCI treatment history,6min walking test,nausea and vomiting,lower limb edema,lung rales,LDL and LVEF are correlaed with bendopnea.The results of time stratification analysis show that the shorter the onset time of symptoms,the higher BNP and the lower LVEF,there were significant statistical differences between the three subgroup.In the second part,120 patients,60 in each group,were enrolled in the study.Before discharge and 6 months after discharge,Minnesota heart failure quality of life questionnaire scores in patients with bendopnea are higher than those without bendopnea,there are statistical differences in BNP,LVEF,New York cardiac function classification,and the number of readmissions within 6 months of discharge,those suggest that the prognosis of heart failure patients with bendopnea is more poor.[conclusion]Bendopnea can be one of the symptoms of assessing heart failure,and predictive assessment of heart failure is more sensitive.The shorter the time required to develop symptoms,the more severe the symptoms of heart failure in patients with heart failure.Bendopnea results in prolonged hospitalization and increases the burden of medical care for the patients.Hypertension and LDL elevation are the independent risk factors for bendopnea in patients with heart failure.Obesity is an influential factor,but not a major risk factor.Bendopnea can affect heart function in patients with heart failure,and can easily lead to increased left and right cardiac load,poor ventricular diastolic function and systolic function in patients with heart failure,resulting in excessive volume load and systemic circulatory congestion.Bendopnea can reflect the volume load in patients with heart failure.Bendopnea has a significant effect on the quality of life in patients with heart failure,and there is a strong correlation between bendopnea and short-term adverse clinical outcomes in patients with heart failure.
