ObjectiveFetal ascites is a common abnormality in prenatal ultrasound screening.It is often considered as one of the types of fetal edema,and its etiology is diversified.Due to the lack of unified diagnostic criteria,mild or temporary ascites may be missed diagnosis,accurate incidence is still difficult to estimate.In recent years,with the enhancement of people's awareness of prepotency,the continuous improvement of prenatal diagnosis technology and the upgrading of image equipment,more and more potential pathological diagnoses have been found.At the same time,with the increasing popularity of new methods of intrauterine treatment of fetuses,such as fetoscopy,ultrasound-guided puncture,intrauterine blood transfusion,Bladder amniotic shunt and so on,intrauterine intervention can be carried out early on the fetus,thus delaying the further deterioration of the disease and creating conditions for postpartum treatment.In this study,62 cases of fetal ascites were analyzed,and their diagnosis,clinical characteristics and prognosis were discussed to explore the effect of intrauterine intervention and perinatal intervention on the survival rate of children,So that we can maximize the survival rate of patients,and provide help for clinical diagnosis and treatment of related cases.MethodsThe clinical data of 62 cases of fetal ascites from June 2013 to June 2018 in obstetrics department of the First Affiliated Hospital of Zhengzhou University were retrospectively analyzed.All patients were diagnosed by ultrasound in our hospital.We collected the patient's related medical history in detail,including history of past illness,traumatic history,history of adverse substance exposure during pregnancy(animals,drugs,poisons,radioactive substances,etc.),complications during pregnancy,history of adverse pregnancy(previous childbearing history of abdominal effusion fetus or abnormal chromosome fetal)and so on.The gestational weeks and methods of termination of pregnancy,laboratory tests,clinical interventions,Apgar score of newborns and weight were recorded,and the long-term prognosis of live births was followed up.Results1.Fetal ascites is one of the clinical manifestations of fetal edema syndrome.The etiology of fetal ascites is diversified.In 62 patients,there were 32 cases(51.6%)patients with ultrasound fetal edema syndrome related and 30 cases(48.4%)patients with fetal ascites who had no other parts of effusion or edema,ascites is simple.2.Patients chose to continue pregnancy in 15 cases,47 cases of patients chose to give up(including 5 cases of intrauterine fetal death).Among the 15 cases of continuing pregnancy,11 cases had normal neonatal development and 4 cases died postpartum.3.Only one case survived in the fetal edema syndrome group and 10 cases survived in the simple fetal ascites group.Conclusions1.The etiology of fetal ascites is diversified.Systematic prenatal examination and multidisciplinary collaborative prenatal consultation can improve the accuracy of etiological diagnosis.2.The survival rate of children with simple fetal ascites is better than that of children with fetal edema syndrome complicated with fetal ascites.3.For patients with fetal ascites,there is no need to terminate pregnancy blindly.Through detailed prenatal examination,etiological analysis and perinatal treatment,some patients can also obtain good pregnancy outcomes.
