Objective To investigate whether lung shadow combined with double-track sign can distinguish the differentiation of benign and malignant.Methods The dates of 771 cases with lung shadow that were confirmed by hispathology and immunohistochemistry after surgery in the first affiliated hospital of Guangxi Medical University of thoracic surgery ward from June2012 to September 2014,426 cases(55.25%)were male and 345 cases(44.75%)were female,with an average age of 56.96±11.00 years old.According to the hispathology results,the patients were divided into the malignant lesion group and the benign lesion group.The relationship between each group and the imaging features was analyzed,as well as the relationship between the lung shadow with or without the dual-track sign and the histopathology of the lung shadow,and the connection between the location of lung shadow with the dual-track sign and the histopathology of the lung shadow.Results Among the 771 cases enrolled in the study,the total number of malignant lesions was 537 cases(69.65%),which included 407 cases(52.79%)of adenocarcinoma and 57 cases(7.39%)of squamous cell carcinoma.The adenocarcinoma and the squamous cell carcinoma.were the main pathological types.The total number of benign lesions was 234 cases(30.35%),including180 cases of inflammatory lesions(23.35%)and 54 cases of non-inflammatory lesions(7.00%).Male and female patients accounted respectively for 55.25%(426/771)and 44.75%(345/771).There were statistically significant differences between malignant and benign lesions among different ganders(X~2=4.01,P=0.045).For lung shadow patients diagnosed with lung squamous cell carcinoma and lung adenocarcinoma by histopathology,there was a statistically significant difference between lung squamous cell carcinoma and lung adenocarcinoma among different ganders(X~2=18.03,P=0.000).Lung shadow can be divided into tumor(>3cm)or nodule(?3cm)according to the maximum diameter.There were statistically significant differences between malignant and benign lesions among pulmonary masses and pulmonary nodules(X~2=6.21,P=0.013).After statistical analysis,the differences between malignant and benign lung shadow lesions with or without spiculation sign,lobulation sign and pleural indentation sign were statistically significant(X~2was respectively59.10,102.11 and 15.44,and all P values were<0.05).Pulmonary shadow was divided into pulmonary mass and pulmonary nodule group according to the maximum length and diameter.According to statistical analysis,there were statistically significant differences between malignant and benign lung mass lesions with or without spiculation sign,lobulation sign and pleural indentation sign in the lung mass group(X~2was respectively 25.40,50.04 and 6.76,and all P values were<0.05).There were statistically significant differences between malignant and benign pulmonary nodules in the pulmonary nodules group(X~2=39.34,55.72,8.24,and all P<0.05)with or without the signs of spiculation,lobulation,and pleural indentation.After statistical analysis,there was no statistically significant difference between benign and malignant lung shadow lesions with or without double-track signs(X~2=0.23,P>0.05).There was no statistically significant difference between benign and malignant pulmonary nodules with or without double-track sign(X~2=0.41,P>0.05).As well as in the pulmonary nodules group(X~2=1.229,P>0.05).There was no statistically significant difference between the different histopathological properties of lung masses with different double-track signs location(X~2=2.71,P=0.766).There were statistically significant differences in different histopathological properties of pulmonary nodules with different double-track signs location.The adjusted residuals were:2.8 in the Same pulmonary and without connected group,3.3 in the Same pulmonary and with connected group,and 1.1 in the With connected and other lung segments none group.Conclusion Adenocarcinoma and squamous cell carcinoma were the most common types of lung cancer.Women are more likely to have malignant lesions than men.The proportion of men with lung squamous cell carcinoma was higher than that of women,and the proportion of women with lung adenocarcinoma was higher than that of men.Lung masses are more likely to be malignant than pulmonary nodules.No matter what the size of lung shadow is,there is a higher probability of malignant lesions in lung shadow with spiculation sign,lobulation sign and pleural indentation sign.When pulmonary nodules were combined with dual-track signs,pulmonary nodules that were not connected to the dual-track signs tended to be inflammatory,pulmonary nodules that were connected to the dual-track signs tended to be more malignant,and pulmonary nodules that were only connected to dual-track signs and other lung segments without lung shadow tended to be more non-inflammatory.When the pulmonary nodule is not connected with the dual-track sign,the possibility of inflammation of the nodule is considered,and surgical treatment is not the first choice.
