ObjectiveTo explore the clinical application of adipose granules combined with fat gel injection in different layers of the face.MethodsWe conducted a study in which 46 female patients who were admitted to the Fifth Affiliated Hospital of Zhengzhou University from December 2016 to April 2018.Patients were divided into two groups:group A(injection of adipose granules in different levels of the face)and group B(injection of adipose granule combined with fat glue in different layers of the face).Under local swelling anesthesia or general anesthesia,the patients received liposuction in the medial and lateral thigh.The fat was divided into two parts for treatment.One was the adipose granules with a granule-sized fat obtained after centrifugation,and the other was the fat gel obtained by repeating cutting with a fat exchanger on the adipose granules.According to the adipose granules and fat gel respectively,the corresponding injection needles and injection levels were selected.Then the uniform,appropriate amount and multi-level regression injection was performed at the site to be filled.High-resolution photos were collected before surgery,1 week after surgery,1 month,3 months,6 months and 12 months after surgery.We evaluated the long-term effects of surgery according to the complications,secondary filling rate and patient satisfaction.Results1.There were no statistically significant differences in age composition,BMI and follow-up time between the two groups(p>0.05).2.In the group A,among the total 11 cases(47.8%),5 cases(21.7%)presented with local unevenness,and 3 cases(13.0%)had subcutaneous hemorrhage.Moreover,subcutaneous induration occurred in 2 cases(8.7%),bilateral asymmetry in 1 case(4.3%).In the group B,there were 1 case(4.3%)with local uneven,1 case(4.3%)with subcutaneous hemorrhage,1 case(4.3%)with bilateral asymmetry,and total 3 cases(13.0%).However,no one(0%)presented with subcutaneous induration.The total complication rate in group A was higher than that in group B(p<0.05).3.There were 9 patients(39.1%)who underwent secondary fat filling in group A,while in group B there were 3 patients undergoing secondary fat filling(13.0%).The secondary filling rate in group A was higher than that in group B(p<0.05).4.In group A,the numbers of patients with very satisfactory were 5(21.7%),12(52.2%)patients with satisfactory,4 cases with generally satisfactory(17.4%),and 2 cases with unsatisfactory(8.7%).Meanwhile,in group B,the numbers of patients with very satisfactory were 16(69.6%),4(17.4%)patients with satisfactory,2 cases with generally satisfactory cases(8.7%),and 1 case with unsatisfactory(4.3%).The overall satisfaction of group B was higher than that of group A(p<0.05).ConclusionAdipose granules combined with fat gel filling at different layers of the face have the advantages of low complication rate,low secondary filling rate and high patient satisfaction.It is an effective method for filling facial volume and realizing facial rejuvenation.
