ObjectiveThis study collected moderately severe chronic periodontitis with occlusal trauma patients with gingival sulcus fluid,then use ELISA kits osteosclerosis in gingival sulcus liquid protein concentration,while detecting the change of periodontal pocket inside bacterial species composition,in order of bacteria and osteosclerosis protein as monitoring and evaluating the feasibility of periodontal health indicators and occlusion treatment effects on both.MethodsBetween February 2018 and July 2018,A total of 40 patients with periodontitis with moderate and severe occlusal trauma were randomly selected and divided into groups A and B,among which occlusal time(OT)was the non-treatment factor.The patients were divided according to the completely random block design principle.Six sites on the buccal and lingual surfaces of each included tooth were selected to collect periodontal bacteria.A,B two groups of plaque samples from 0 d(baseline)and gingival sulcus liquid,the bacteria monitor and periodontal clinical index,index of occlusion,check,and then two groups for SRP treatment at the same time,in group B T-scanⅢ type analysis system under the guidance of occlusion treated with occlusal adjustment.During the 3-month period of the two groups,bacterial monitoring,gingival crevicular fluid collection and periodontal clinical indicators were performed for each group every month,and occlusal adjustment therapy was performed for group B once a month.After each collection of plaque samples,phase difference microscope was used for bacterial detection.After the complete collection of samples,enzyme linked immunosorbent assay(ELISA)was used to detect the osclerotic protein concentration in gingival sulcus fluid,and statistical methods were used for statistical processing of all data.ResultsThe osclerotic protein level in gingival crevicular fluid of the occlusal adjustment treatment group(group B)was significantly lower than that of the non-occlusal adjustment treatment group(group A)in each time period,and the number of cocci was significantly higher than that of the non-occlusal adjustment treatment group(group A),and the results were statistically significant(P<0.05).And over time,the number of cocci in group A tended to the baseline level,the number of cocci in group B reached A healthy level two months later(69%),and the number of cocci basically remained at this level at the third month(73%).Meanwhile,osclerotic protein level in gingival crevicular fluid was positively correlated with periodontal clinical indicators AL,BI and PD(P<0.001),and the correlation was strong.Conclusions1.Occlusal adjustment treatment can reduce the concentration of osclerotic protein in gingival crevicular fluid of patients with chronic periodontitis and occlusal trauma,suggesting that it may help to inhibit the destruction of periodontal bone tissue.2.Occlusal adjustment therapy can increase the number of cocci in gingival crevicular fluid in patients with chronic periodontitis and occlusal trauma,suggesting that it may help to reduce the infection of opportunistic pathogens in periodontal tissues.3.The clinical application of bacterial analysis is simple,rapid and more objective,so it should be considered as an indicator of periodontal health,not only for diagnostic purposes,but also to monitor the success of treatment strategies.4.Osclerotic protein level in human gingival crevicular fluid can be considered as a potential indicator for the diagnosis and treatment of periodontal disease.5.The concentration of SOST in gingival crevicular fluid of patients with chronic periodontitis combined with occlusal trauma was positively correlated with PD,BI and AL.
