Abstract/Summary:
ObjectiveBy summarizing the history,clinical manifestations,laboratory results,and ankle joint MRI sequences of patients with ankylosing spondylitis,and retrospectively analyzing the relationship between the existence of sacroiliac joint magnetic resonance fat backfilling and the clinical characteristics,activity and severity of ankylosing spondylitis,the purpose is to discuss the diagnostic value and significance of fat backfilling in ankylosing spondylitis.MethodsRetrospective analysis of patients with ankylosing spondylitis who were admitted to the First Affiliated Hospital of Zhengzhou University from January 2013 to December 2016.404 patients were selected as subjects.The patients included in the study were in compliance with the proposal for modification of the New York criteria in 1984.The clinical data of 307 cases of ankylosing spondylitis with fat backfill and 97 cases of ankylosing spondylitis with non-fat backfill were analyzed.The measurement data were analyzed by rank sum test.The qualitative data were analyzed by χ2 test and logistic regression analysis.A statistical difference was considered at p < 0.05.ResultsThere were 307 patients in the fat backfill group,218 males and 89 females,male: female = 2.45:1,median age of onset: 30 years old,the median duration of disease was 4 years,and the median number of peripheral joint involvement is 1.There were 99 patients in the non-fat back-filled group,54 males and 43 females,male: female = 1.26:1,median age of onset: 32 years old,the median duration of disease was 2 years,the median number of peripheral joint involvement is 1.The proportion of men in the fat backfill group was significantly higher than that in the non-backfill group,the age of onset of fat-filled group was not statistically significant compared with non-fat back-filled group(P >0.05).The time-long backfill group was significantly higher than the non-backfill group,and the difference was statistically significant(P=0.044).Compared with the non-fat back-filled group,there was no statistical difference in the extra-articular involvement of the kidney,skin,lungs,and intestines with the fat backfill group.Laboratory tests: red blood cell distribution width,CRP,D-Dimer level in the fat backfill group were more likely to rise than the non-fat backfill group,and the difference was statistically significant(P=0.018,P=0.000,P=0.001).There was no significant difference in X-ray classification of bilateral sacroiliac joints between the two groups(P > 0.05).Logistic regression analysis was performed on the significant difference index in the fat backfill group,CRP,D-dimer and male were risk factors for backfilling.The OR values respectively were 1.009(1.000-1.017),2.564(1.274-5.16)and 1.832(1.079-3.108).ConclusionThe course of disease,RBC distribution width,CRP and D-dimer levels in the fat backfilling group were significantly higher than those in the non-backfilling group,and CRP,D-dimer and males may be risk factors for fat backfilling.
