Background and objectivesMagnesium ion(Mg2+)plays an important role in maintaining the life and health of the body.In preterm infants,the protective effect of prenatal Mg2+supplementation on the nervous system of preterm infants has been widely recognized.Researchers begin to pay attention to the effects of serum Mg2+level on the development of nervous system and non-nervous system in newborn.We aim to explore the correlation between the level of serum magnesium ion(Mg2+)and diseases in neonates within 24hours after birth,accordingly provide a scientific basis for clinical improvement of neonatal survival condition.MethodsThis retrospective study included neonates admitted to the neonatology department of the First Affiliated Hospital of Zhengzhou University within 24 hours after birth from September 2018 to December 2018 and whose blood gas with Mg2+,heart and brain doppler ultrasound were examined within 24 hours.Exclude the following neonates:1.Neonates with congenital malformations,chromosomal abnormalities or congenital metabolic defects;2.Neonates with renal insufficiency;3.Basic data were incomplete.According to the neonatal serum Mg2+level,they were divided into three groups:group A:?0.51mmol/L?lower quartile??56 cases?,group B:>0.51mmol/L to?0.65mmol/L?interquartile range??97 cases?and group C:>0.65mmol/L?upper quartile??48 cases?,using group B as reference,adjusting birth weight and gestational age for logistic regression analysis of early neonatal diseases.Results1.1 The comparison of different serum Mg2+levels and the general situation of pregnant mothersThere were no significant differences in maternal age,mode of delivery,gestational diabetes,gestational hypertension,placental previa,amniotic fluid contamination,premature rupture of membranes incidence in the three groups?P>0.05?.1.2 The comparison of different serum Mg2+levels and the general situation of the newbornThe birth weights of the three groups were 2190?1525,2750?g,2100?1430,2580?g,2115?1625,2638?g,and the gestational age of the three groups was 34.0?32.0,37.0?weeks,?31.1,35.6?weeks,34.3?31.9,35.9?weeks respectively;the differences in birth weight and gestational age between the groups were statistically significant?P<0.05?.There were no significant differences in gender,blood pressure,temperature,pulse,breathing rate between the groups?P>0.05?.1.3 Effects of different serum Mg2+levels of early newborns on diseases developmentThe incidence of brain injury immediately after birth in the three groups was71.4%,47.4%,and 70.8%,respectively.The incidence of early pneumonia in the three groups was 19.6%,5.2%,and 6.3%,respectively.The incidence of early respiratory failure were 58%,35%,and 52%,respectively.There were significant differences in the incidence of brain injury,pneumonia,and respiratory failure between the groups?P<0.05?.There was no significant difference in the incidence of patent foramen ovale,Patent ductus arteriosus,heart failure,Meconium aspiration syndrome,respiratory failure and respiratory distress syndrome between the groups?P>0.05?.1.4 Regression analysis of serum Mg2+levels and early diseases adjusting birth weight and gestational ageAfter adjusting birth weight and gestational age,the incidence of brain injury was significantly higher in group A and group C than in group B?AOR=2.488 95%CI:1.489-4.156,AOR=1.934 95%CI:1.125-3.326?;Compared with group B,the incidence of neonatal pneumonia was higher in group A and group C?AOR=1.10795%CI:0.452-2.714,AOR=2.29 95%CI:1.008-5.203?.Compared with group B neonates,the incidence of neonatal respiratory failure was higher in group A and group C?AOR=2.641 95%CI:1.647-4.234,AOR=1.295 95%CI:0.777-2.160?.Conclusions1?The neonatal serum Mg2+level within 24 hours after birth was correlated with birth weight,gestational age and the incidence of pneumonia,brain injury and respiratory failure.2?There is a"U"-type correlation between the level of neonatal serum Mg2+and the incidence of brain injury,pneumonia and respiratory failure within 24 hours after birth.
