Background and ObjectiveMigraine is the most common type of primary headache in the clinic.The headache is often repeated,and the pain is usually on one side or on both sides.Most of them are moderate to severe pulsatile pain.Headache attacks are often accompanied by autonomic nerve symptoms such as nausea,vomiting,and fearing of sound.Headache can be induced or aggravated by external sound,light stimulation,and the like.Because of its high incidence and severe headache,it often has an important impact on the life,study or work of the patient.It has become one of the seventh most common diseases in the world.Although many studies on migraine have been conducted over the years,the etiology and pathogenesis are still unclear.Aseptic inflammation around the trigeminal nerve is one of the most recognized pathogenesis of migraine.The theory suggests that aseptic inflammation causes migraine attacks,in which activation of the trigeminal nervous system is a key link.The study found that homocysteine(HCY),neutrophil to lymphocyte ratio(NLR)may be related with the migraine,but not clear,and the mechanism of migraine attack is not clear too.HCY is a sulfur-containing amino acid in human blood.High HCY is associated with vascular endothelial injury,atherosclerosis and ischemic cardiovascular and cerebrovascular diseases,which cause inflammation and oxidative stress through a series of pathways.As a common and readily available hematological marker,NLR combines inflammatory cell components of neutrophils and lymphocytes,it is found to be associated with the occurrence and prognosis of various diseases through inflammatory pathways,but whether both of them are related with the migraine,it is still unclear.At home and abroad,the research between migraine and HCY is still inconclusive.There are few studies on migraine and NLR.In view of this,this study aims to study the changes of HCY and NLR in blood of migraine patients,discovering its role that they are in the mechanism of migraine.To provide a factual basis for early prevention and better treatment of migraine.Materials and MethodsThirty patients diagnosed with migraine attacks who came from the neurology clinic or hospitalization of our hospital from December 2016 to October 2017,as the migraine attacks.After the headaches and other symptoms were controlled of the thirty patients with migraine attack,and the patients were followed up for 1 month,as the migraine intermittent group.The healthy subjects who underwent physical examination in our hospital in the same period were the control group.In the migraine attack group and the control group,blood samples were taken from the peripheral venous,the blood routine and blood homocysteine test were taken at the time of visiting the doctor.Both groups obtained neutrophil count(NC)and lymphocyte count(LC),HCY,and calculate the NLR.Patients in the migraine intermittent group obtained the above indicators after one month using the above method.Ask and record the number of headache episodes one month before visiting the doctor this time and measure the degree of headache in this time.Statistical analysis with SPSS22.0 software.Results1.HCY content migraine attack group and migraine intermittent group are significantly higher than the control group,the difference is statistically significant(P<0.05),HCY content migraine attack group is higher than migraine intermittent group,but the difference is not statistical significance(P>0.05);2.The NLR value migraine attack group is significantly higher than the control group and the migraine intermittent group,the difference is statistically significant(P<0.05),and the migraine intermittent group NLR value is slightly higher than the control group,the difference is not statistically significant(P> 0.05);3.There is a positive correlation between the frequency of headache and the degree of pain and the NLR.The difference is statistically significant(P<0.05).The frequency of headache attack and the degree of pain are not correlated with HCY.The difference is not statistically significant(P>0.05).Conclusions1.HCY content increased during migraine attack and migraine interval,and migraine attack period is slightly higher than migraine interval,suggesting that HCY may be involved in the pathogenesis of migraine,hyperhomocysteinemia may be one of the risk factors for migraine;2.The NLR value is significantly higher in the migraine attack period than in the migraine interval and healthy controls,and the NLR value migraine interval is slightly higher than the healthy controls,suggesting that inflammation may be involved in the pathogenesis of migraine;3.The higher the NLR value,the higher the frequency of headache attacks and the greater the pain.NLR may become an indicator of inflammation in predicting the prognosis of migraine.
