Objective The partial pressure of carbon dioxide in artery(PaCO2)is an important indicator to measure alveolar ventilation and evaluate the acid-base balance.The transcutaneous carbon dioxide tension(TcCO2)can finish the same work.In clinical work,the veracity of TcCO2 has been verified by a large number of studies,but the factors affecting the accuracy of TcCO2 are not completely clear.Methods Total 60 children were collected who hospitalized in neonatal intensive care unit(NICU).Including 17 cases of PaCO2 were less than 35 mmHg,24 cases were greater than 45 mmHg,and 21 cases were between 35 and 45 mmHg.All the children were treated by mechanical ventilation.The corresponding gestational age,birth weight,mean arterial pressure,heart rate,perfusion index,critical mass score,PaCO2 and TcCO2 were collected,collated and systematically analyzed to evaluate the accuracy of TcCO2 and its influencing factors.Results There was a highly positive correlation between PaCO2 and TcCO2,the correlation coefficient was 0.969,P value was less than 0.05.PaCO2=6.163+0.853×TcCO2 was further established by regression analysis.There was a low positive correlation between PaCO2 and TC-PA(The difference between the two TcCO2 and PaCO2),the correlation coefficient was 0.335,P value was less than 0.05.Further regression analysis of regression equation was established:TC-PA=-3.756+0.1×PaCO2.There was a low negative correlation between PI(perfusion index)and TC-PA,the correlation coefficient was-0.266,P value was less than 0.05.Further regression analysis of regression equation was established:TC-PA= 10.039-10.23xPI.There was no correlation between TC-PA and gestational age,birth weight,mean arterial pressure,heart rate,critical mass score,P values were greater than 0.05.Conclusions The PaCO2 from the children who were treated by Mechanical ventilation can be accurately predicted by transcutaneous carbon dioxide monitoring.The gestational age,birth weight,mean arterial pressure,heart rate,critical mass score,had no significant effect on the accuracy of TcCO2,while PaCO2 and PI had impacts on the accuracy of TcCO2.
