Objective:To evaluate and analyze the effects of urinary kallidinogenase,butylpht-halide soft capsule and combination therapy in treatment of acute ischemic stroke.Methods:This study is a retrospective study.All selected cases were from patients with acute ischemic stroke who were admitted to the dep artment of neurology of the third affiliated hospital of Nanchang University for the first time from February 2014 to October 2018.All the selected patients met the diagnostic criteria for acute ischemic stroke,with the onset age ranging from 20 to 80 years old and the onset time within 7 days.To selected patients grouped into conventional treatment group(A0 group and experimental group(on the basis of conventional treatment,respectively,with urinary kallidinogenase injection A1,A2,butylphthalide soft capsule group,urinary kallidinogenase injection joint butylphthalide soft capsule group A3),and will be A1,A2,the A3 group according to TOAST classification can be divided into: for artery atherosclerosis sex(A1a group,both A2 a,A3a group),cardiac(A1b,A2 b group,A3 b group),small artery disease degeneration(A1c,A2 c group,A3 c group).All patients underwent acute national institutes of health neurologic impairment score(NIHSS)before treatment and at 7 and 14 days after treatment.To analyze and compare the effects of different treatment schemes on acute cerebral infarction and cerebral infarction of different etiology.Results:1.Analysis of the therapeutic effect of different drugs on cerebral infarction: analysis of A0,A1,A2 and A3 groups within each group: the mean NIHSS score of the four groups at 7 and 14 days after drug treatment was lower than that before treatment,and the difference between the NIHSS score after treatment and that before treatment was statistically significant(P < 0.05).2.The total effective rate of different drugs was compared: the difference between A1,A3 group and A0 group was statistically significant(P < 0.05),while the difference between A2 group and A0 group was not statistically significant(P > 0.05).There was no statistical significance between groups A1,A2 and A3(P > 0.05).3.To compare the effects of different drugs on cerebral infarction: The difference of NIHSS scores between A0 group and A1 group was statistically significant before treatment(P < 0.05),and the mean score of A1 group was higher than that of A0 group,and the NIHSS score was not statistically significant on the 7th day and the 14 th day of treatment(P > 0.05).There was no significant difference in NIHSS scores between A1 and A3 groups before treatment,7 days after treatment,and 14 days after treatment(P > 0.05).There was a statistically significant difference in NIHSS scores between A0 and A3 groups before treatment(P < 0.05),and the mean value of A3 group was higher than that of A0 group,and there was no statistically significant difference in NIHSS scores between 7 and 14 days after treatment(P > 0.05).There were statistically significant differences in NIHSS scores between A0 and A2 groups,A1 and A2 groups,and between A2 and A3 groups before treatment,7 days after treatment,and 14 days after treatment(P < 0.05).4.The efficacy of different drugs in the treatment of atherosclerotic cerebral infarction was compared: the differences in NIHSS scores between A1 a and A2 a groups,between A2 a and A3 a groups before treatment,7 days after treatment,and 14 days after treatment were statistically significant(P < 0.05).There was no significant difference in NIHSS scores between A1 a and A3 a groups before treatment,7 days after treatment,and 14 days after treatment(P > 0.05).5.The efficacy of different drugs in the treatment of cardiogenic cerebral infarction was compared: there was no statistically significant difference in score between the groups before treatment and 7 days after treatment(P > 0.05).Compared with the A3 b group,the NIHSS score of A3 b group on the 14 th day of treatment was lower and statistically significant(P < 0.05).There was no significant difference between the other groups(P > 0.05).6.To compare the efficacy of different drugs for small artery occlusive cerebral infarction: the difference of NIHSS score between A1 c and A2 c group before treatment and 7 days after treatment was statistically significant(P < 0.05),but no statistically significant difference after 14 days of treatment(P > 0.05).There was no significant difference in NIHSS scores between A1 c and A3 c groups before treatment at 7 days and 14 days after treatment(P > 0.05).There was a statistically significant difference in NIHSS scores between A2 c and A3 c groups before treatment(P < 0.05),but no statistically significant difference between 7 and 14 days after treatment(P > 0.05).Conclusion:1.Urinary Kallidinogenase injection and butylphthalide soft capsule can significantly improve the neurological injury in patients with acute ischemic stroke,and are superior to the conventional treatment group.2.For atherosclerotic cerebral infarction,there was no significant difference in the efficacy between Urinary Kallidinogenase,injection and Urinary Kallidinogenase,injection combined with butylphthalide soft capsule,and there was no difference in the efficacy between the two treatments,which may be related to the inclusion of data and other factors.3.For cardiogenic cerebral infarction,the efficacy of Urinary Kallidino-genase,injection combined with butylphthalide soft capsule was better than that of Urinary Kallidinogenase,injection,which may be related to the protective effect of butylphthalide on neuronal mitochondria.4.For the denaturalized cerebral infarction caused by arteriolar disease,the combination of Urinary Kallidinogenase,injection and Urinary Kallidinogenase,injection combined with butylphthalide soft capsule was superior to butylphth-alide soft capsule.
