Objective:To investigate whether there is a correlation between serum vitamin D levels and indicators of chronic heart failure in the elderly,and whether vitamin D supplementation has an effect on chronic heart failure related indicators in the elderly,thus providing a new idea for clinical treatment of chronic heart failure in the elderly.Method:A total of 100 elderly patients with chronic heart failure with 25(OH)D<30ng/m L diagnosed and treated in the outpatient department and inpatient department of the Geratology at Yijishan hospital,affiliated to Wannan Medical College from July 2017 to July 2018 were collected.The patient's general information,laboratory examination and cardiac ultrasound were recorded.According to the patient's serum 25(OH)D level,the vitamin D deficiency group,the vitamin D deficiency group and the vitamin D severe deficiency group were compared.Left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),N-terminal B-type natriuretic peptide(NT-pro BNP),serum calcium ion,6-minute walking distance,C-reactive protein(CRP)and other indicators The difference was analyzed between serum 25(OH)D levels and LVEF,LVEDD,NT-pro BNP,serum calcium,6-minute walking distance,CRP and otherindicators in elderly patients with chronic heart failure.The above 100 patients were divided into two groups according to the randomized and single-blind control principles.Both groups were treated with conventional anti-heart failure drugs.The supplement group received oral administration of calcitriol capsules(0.25 ug/d)on the basis of conventional treatment.After 12 weeks,the serum 25(OH)D levels of the two groups were compared,LVEF,LVEDD,NT-pro BNP,serum calcium ion,6-minute walking distance and CRP were statistically different before and after treatment;and 25(OH)D levels,LVEF,LVEDD,NT-pro BNP,and after treatment Serum calcium ion,6-minute walking distance and CRP changes were compared between groups to see if there was any statistical difference.Result:With the continuous decrease of serum 25(OH)D level,the patients' LVEF,6-minute walking distance and serum calcium ion value showed a decreasing trend,the difference was statistically significant(P<0.01);NT-pro BNP,LVEDD and CRP showed a gradual increase trend,and the difference was statistically significant(P<0.01).Linear correlation analysis showed that LVEF,6-minute walking distance,serum calcium ion value was positively correlated with serum 25(OH)D,and NT-pro BNP,LVEDD and CRP were negatively correlated with serum 25(OH)D.In the randomized,double-blind,controlled trial,the mean serum 25(OH)D levels and serum calcium levels were significantly higher in the treatment group compared with those before 12 weeks(P <0.01),while the mean 25(OH)D levels and serum calcium ions were higher in the control group.No significant change(P>0.05).The LVEF value and the 6-minute walking distance of the two groups were significantly increased after 12 weeks,the difference was statistically significant(P<0.05).The NT-pro BNP,LVEDD and C-reactive protein in the two groups were obvious after 12 weeks.Decreased,the difference was statistically significant(P<0.01),However,LVEF and 6-minute walking distance in the supplemental group were significantly higher than those in the control group,and the values of nt-probnp,LVD and c-reactive protein were significantly lower than those in the control group,with statistically significant differences(P<0.05).Conclusion:1.There is a correlation between serum 25(OH)D level and heart failure related indicators in elderly patients with chronic heart failure,which is positively correlated with LVEF,serum calcium ion and 6-minute walking distance,and LVEDD,NT-pro BNP,and C-reaction.The protein is negatively correlated.2.Appropriate supplementation of calcitriol can improve the vitamin D deficiency status of elderly patients with chronic heart failure,as well as improve the therapeutic effect of heart failure.
