Effects Of Non-digestive Medical Consultations On Dyspeptic Symptoms And Quality Of Life In Patients With Functional Dyspepsia

Objective:To investigate the effects of non-digestive medical consultations on dyspeptic symptoms and quality of life in patients with functional dyspepsia(FD)due to one or more somatic symptoms.Methods:399 FD patients who met Rome IV criteria were collected.DSS,PHQ-15,adapted PHQ-15,GAD-7,PHQ-9 and NDI were used to evaluate the severity of dyspepsia,somatic symptoms,anxiety,depression and NDI of FD patients.Focus on the diagnosis and treatment with FD patients who have been treated in non-digestive medical consultations.Spearman and Pearson correlation analysis,chi-square test,one-way ANOVA,ordered logistic regression and multiple linear regression were used to analyze the relationship between somatization,anxiety and depression of the FD patients due to one or more somatization symptoms in non-digestive medical consultations,as well as the influence on DSS and NDI.Results:The prevalence of non-digestive medical consultations in patients with FD who were included in the study was 55.4%.The gender was related to the number of departments visited(χ~2=11.964,P=0.008)and the number of diseases diagnosed(χ~2=10.991,P=0.012)of FD patients.The FD subgroup was not related to the number of departments visited(χ~2=8.731,P=0.189)and the number of disease diagnosed(χ~2=11.902,P=0.064).With the increase in the the number of departments visited and the number of diseases diagnosedin FD patients,PHQ-15,adapted PHQ-15,anxiety and NDI showed an increasing trend among the groups,and the difference was statistically significant.DSS showed a decreasing trend with the increase of the number of departments visited and the number of diseases diagnosed,but there was no significant difference among the groups.There was no statistically significant difference in depression scores between groups.The DSS and NDI scores(mean±SD)of the patients in the overlapping group were highest among the FD subtypes.Among the total FD patients,gender(r=0.154,P=0.002;r=0.145,P=0.004),age(r=0.110,P=0.029;r=0.134,P=0.007),NDI(r=0.144,P=0.004;r=0.159,P=0.001),duration of disease(r=0.134,P=0.007;r=0.120,P=0.017),PHQ-15(r=0.309,P=0.000;r=0.255,P=0.000),adapted PHQ-15(r=0.332,P=0.000;r=0.252,P=0.000),anxiety(r=0.173,P=0.001;r=0.154,P=0.002)were related to the number of departments visited and the number of diseases diagnosed,respectively;there was no significant difference in DSS and depression between the two groups.PHQ-15(r=0.518,P=0.000),adapted PHQ-15(r=0.446,P=0.000),anxiety(r=0.277,P=0.000),depression(r=0.483,P=0.000)were correlated with DSS.The number of departments visited,the number of diseases diagnosed and DSS were not statistically significant.PHQ-15(r=0.377,P=0.000),adapted PHQ-15(r=0.373,P=0.000),anxiety(r=0.437,P=0.000),depression(r=0.520,P=0.000),the number of departments visited(r=0.144,P=0.004)and the number of disease diagnosed(r=0.159,P=0.001)were all related to NDI.Ordered logistic regression analysis was performed on the diagnosis and treatment of non-digestive medical consultations in FDpatients.The results showed somatization(OR=0.272,P=0.000),anxiety(OR=0.100,P=0.014)were the influencing factors of the number of departments visited.Somatization(OR=0.119,P=0.002),anxiety(OR=0.055,P=0.032),NDI(OR=0.020,P=0.005)were the influencing factors of the number of diseases diagnosed.Linear regression of DSS and NDI,somatization(β=0.251,P=0.000),depression(β=0.339,P=0.000)were the influencing factors of DSS.Anxiety(β=0.169,P=0.002),depression(β=0.414,P=0.000)and the number of disease diagnosed(β=0.119,P=0.005)were the influencing factors of NDI.Conclusion:The gender,somatization,anxiety and NDI of FD patients were correlated with the number of departments visited and the number of diseases diagnosed.Non-digestive medical consultations of FD subtype EPS patients affected DSS and NDI.These results suggest that non-digestive medical consultations in FD patients impaire the quality of life.If we manage the related factors of non-digestive medical consultations,it may further improve the quality of life of FD patients.
