| ObjectiveThe purpose of this study is to preliminarily explore the effect of Finger Stimulate-Irritational Therapy(FSIT)in the treatment of cerebral palsy related talipes valgus(CPRTV),so as to provide basis and direction for further research on the effect and mechanism of FSIT in the treatment of talipes valgus in children with cerebral palsyMethods From January 1,2018 to November 30,2018,a total of 40 cases of cerebral palsy with bilateral talipes valgus were enrolled in the study,aged 24-65 months,with an average age of(40.73±14.996)months.The children were selected and treated in the outpatient or inpatient of rehabilitation medicine department.The enrolled subjects with cerebral palsy related talipes valgus were recruited according to the same inclusion/exclusion criteria and randomly divided into 2 subgroups:conventional therapy group(control group,n=20)and finger stimulate-irritational therapy group(case group,n=20).The children in the control group received routine kinesitherapy,while the children in the case group received 20 minutes of routine kinesitherapy and 20 minutes of FSIT.All children are received a total of 4 weeks of rehabilitation treatment.General clinical data and sEMG of lower limb were collected at the time of enrollment,and sEMG of lower limb was assessed only at the end of follow-up.sEMG signals of bilateral tibialis anterior muscle,medial head of gastrocnemius muscle and peroneus longus muscle were collected with delsys surface electromyography tester,the signals were recorded for 30 seconds continuously and tested for 3 times.Electromyographic signal for 40-500 Hz band pass filter process.The integrate electromyographic(iEMG)and root mean square(RMS)of the stable waveforms 4-5s,14-15s and 24-25s were obtained,and the mean value of the parameters collected three times was taken for statistical analysis.SPSS 20.0 was used for statistical analysis on the data.The data were not normally distributed.The median of sEMG parameters and improvement rate were tested by rank-sum test before and after treatment,and P<0.05 was considered statistically significant.ResultsAfter 4 weeks of treatment,the RMS and iEMG values of bilateral tibialis anterior muscle and medial gastrocnemius muscle of the two groups were higher than those before treatment,and the values in the case group were significantly higher(In the control group,the improvement rates of RMS and iEMG of tibial anterior muscle,medial gastrocnemius muscle,peroneus longus muscle were 2.00×10-1,7.77×10-1,1.53×10-1,2.06×10-1,0.94×10-1,0.53×10-11 and the improvement rates of RMS and iEMG of that were 23.75×10-1,43.98×10-1,-1.22×10-1,22.81×10-1,21.17×10-1,-2.06×10-11 in the observation group,respectively.P<0.05).The RMS and iEMG values of bilateral peroneal longus muscle in the case group were lower than those in the control group,and the relevant indicators in the case group were lower than those before treatment but were higher in the control group.The differences were statistically significant(P<0.05).ConclusionsThe curative effect of FSIT in the treatment of cerebral palsy related talipes valgus is better than that of regular therapy.After treatment,the muscle strength and tension of tibialis anterior muscle and gastrocnemius muscle in children are higher than that of before,while the muscle tension of fibula longus muscle are lower.Therefore,FSIT is an effective conservative treatment for talipes valgus,which efficacy can be further explored and clinically promoted. |
