Background and purposeOral lichen planus(OLP)is a chronic inflammatory autoimmune disease involving stratified squamous epithelium.Erosive OLP is the most harmful in clinic.At present,immunomodulatory drugs are the first choice in clinic.Glucocorticoid,as a common drug,can only relieve symptoms.Due to the long-term use of hormones has obvious side effects,and local administration process will also bring discomfort to patients,resulting in psychological fear.Therefore,the exploration of new treatments has become the direction of attention of researchers.Total glucosides of paeony capsule(TGP)is an immunomodulatory drug concerned by domestic scholars in recent years.it has been widely used in the treatment of autoimmune diseases and has obvious curative effect.Low power laser has the effect of biological stimulation,which can increase cell activity,improve microcirculation and promote tissue healing.it is a new progress in the treatment of oral mucosal diseases.This study will evaluate the clinical efficacy of semiconductor laser combined with TGP in the treatment of erosive OLP,and find a new method for the clinical treatment of erosive OLP.Materials and methodSixty-four patients with erosive OLP were randomly divided into experimental group(n = 32)and control group(n = 32).Both groups were treated with TGP,meanwhile the experimental group was combined with semiconductor laser irradiation for twelve weeks.The score of the clinical symptoms(the degree of pain)and clinical signs,the status of adverse reactions and the recurrence during follow-up were recorded by the same stomatologist before and after two,four and twelve weeks of treatment.To evaluate the clinical efficacy,total effective rate,adverse reaction rate and recurrence rate of the two groups.Result1.The degree of pain was compared between the two groups before treatment and two weeks,four weeks and twelve weeks after treatment,respectively,and there was significant difference between the two groups(P<0.05).2.The clinical signs of the two groups were compared within and between groups before treatment and two weeks,four weeks and twelve weeks after treatment,respectively.The results showed that there was significant difference between the two groups(P<0.05).3.The total effective rate was 90.6%in the experimental group and 59.4%in the control group(P<0.05).The total effective rate was 90.6%in the experimental group and 59.4%in the control group(P<0.05).4.During the treatment period,there were three cases of diarrhea and eight cases of adverse reactions in the two groups.The adverse reaction rate was 15.6%in the experimental group and 18.7%in the control group.There was no significant difference between the two groups(P>0.05).5.Three months after treatment,the cured and effective patients in the two groups were followed up.The recurrence rate was 8.33%in the experimental group and 14.28%in the control group.There was no significant difference between the two groups(P>0.05).Conclusion1.Semiconductor laser combined with TGP in the treatment of erosive lesions of OLP has achieved good results,which can quickly relieve pain and promote the healing of erosive surface.2.The short-term efficacy of semiconductor laser combined with TGP in the treatment of erosive OLP is stable and the adverse reactions are less.it can be used as a new clinical method for the treatment of erosive OLPBackground and purposeHereditary gum fibromatosis(HGF)is a rare benign lesion characterized by slow fibrous hyperplasia of gum tissue.The etiology of the disease is not completely clear,has a family history,mainly autosomal dominant inheritance.At present,it is reported that there are five subtypes of HGF,of which SOS1 and REST are identified as pathogenic genes,and other pathogenic genes are located on 5ql3-q22,2p23.3-p22.3 and 11p15.In this study,a pedigree of HGF was collected to identify the pathogenic genes of HGF in this pedigree to provide genetic advice for antenatal diagnosis.Materials and methodA patient with HGF was diagnosed by medical history collection,clinical examination and imaging examination accepted the sequence therapy was mainly performed by semiconductor laser resection.In addition,we further study the drawing of pedigree.With the informed consent of the family members,the peripheral venous blood of four family members was extracted and the DNA genome was extracted.Screening of mutant genes by next generation sequencing(NGS),sanger sequencing to verify the presence or absence of pedigree co-isolation,bioinformatics prediction of harmfulness,and other methods to identify the pathogenic gene of the HGF family mutation.Results1.There were no mutations in SOS1 gene and REST gene in this family,and there were no mutations in 5q13-q22,2p23.3-p22.3 and 11p15.2.According to the mutation gene and protein interaction gene binding ACMG standard of single nucleotide variant(SNV)and Insertion-deletion(InDel)in NGS,a total of twenty-eight candidate genes were screened out according to the harmful classification of mutation sites.3.Through the sanger sequencing of four family members,it was identified that only the MRGPRX1 gene located in 11p15.1 and the G6PD gene mutation site located on the long arm of X chromosome conformed to the phenotypic co-segregation of the family..Conclusion1.The pathogenic gene of this family is not one of the five subtypes of HGF reported,and it is a new pathogenic gene that has not been reported.2.G6PD gene may not be the pathogenic gene of HGF in this family,but the proband is a carrier of G6PD deficiency,so it is necessary to make antenatal diagnosis and pay attention to prevention in the future.3.MRGPRX1 gene may be the pathogenic gene of HGF in this pedigree.It needs to be further verified by increasing the sample size and function of the pedigree.
