BackgroundWith the increasing growth of hemodialysis patients and popularization of hemodialysis technology,the hospitalrs hemodialysis center has become increasingly saturated from the top three hospitals to community health service centers.Meanwhile,the independent dialysis centers also arises at the historic moment,it is obvious that hemodialysis technology have been fully developed and the hemodialysis patients transfer from superior hospitals to grass-root ones,as a results of realizing nearby treatment.However,evaluating the level of medical services,people often pay too much attention to technology and neglect the research on humanistic care,among which the privacy protection of hemodialysis patients is one kind of humanistic care.Despite that nearby treatment brings convenience to patients,it is also easy to expose hemodialysis patient's privacy.With the developments of human civilization and enhancements of people's law consciousness,they are paying more attention to their legal rights and interests protection.The right of privacy is the developing personal right and become inviolable for people's basic needs.So patients' privacy protection awareness is improving,which has urged the medical institutions to pay attention to protect personal privacy of patients.Objectives(1)To investigate the status quo of privacy protection provided by hemodialysis nurses and the privacy protection requirements of hemodialysis patients;(2)To explore the influence factors of hemodialysis nurses on cognition and behavior of privacy protection;(3)To explore the influence factors of hemodialysis patients on need of privacy protection;(4)To analyze the correlation between hemodialysis nurses,cognition of patients'privacy and patients' requirements;MethodsHemodialysis nurses from 78 hospital nationwide and patients from two top hospitals were investigated in our study.Demographic data and privacy protection data of nurses were collected,as well as those in patients.Taking the Chinese version of patients' privacy scale as the template,the hemodialysis patients,privacy scale was adjusted and tested for reliability and validity to make it suitable for investigating the privacy status of hemodialysis patients.The privacy status of nurses and patients was investigated using the hemodialysis patient privacy scale.Result1.A survey in 78 hemodialysis nurses across the country found that patients,,body privacy","personal information"and''disease-related information" were generally considered by nurses as patient privacy.The common ways to expose patients,privacy are"changing dressing for catheterization patients","insufficient human resources of nurses" and "lack of relevant system about patients,privacy protection in departments".Hemodialysis nurses'" cognition"about patients' privacy protection is superior to"behavior" of nurses.Through linear regression analysis,it is found that "age","training experiences","training willing" is the impact factors of hemodialysis nurses' eognition to patients privacy protection.There was a negative correlation between"age"and"nurses cognition".And there were positive correlation among"clinical working time","training experiences","training willing"and the nurses' cognition."Post","training experience"and"knowledge of laws and regulations"were the impact factors for hemodialysis nurses to protect patients,privacy.Among them,the scores in the nursing directors were lower than that in non-nursing directors,and there was a positive correlation between" training experience","knowledge of laws and regulations" and "behavior of nurses".2.The survey of 102 hemodialysis patients in two third-class hospitals found that 68.63%of the patients believed that personal privacy should be protected in the medical environment." personal basic information"was the primary privacy content needed to be protected for major patients.Using electronic medical record system was the most likely way to expose patients,privacy by nurses.The scores of patients,"demand"and "satisfaction" in each dimension were compared,and it was found that there was no significant difference in the total score and other dimensions except the score of" gender privacy" and "environmental privacy".The results of multiple linear regression analysis showed that "age ","religious belief" were the impact factors of patients' requirements,among which" age"and"requirements" presented negative correlation,furthermore,patients with religious beliefs had a greater demand for privacy protection.3.The scores of"nurses' cognition" and "patients'needs","nurses,behavio" and"patients' satisfaction" were compared respectively.The cognitive score of hemodialysis nurses on patients'privacy protection was higher than that of patients,requirements,which showed a significant difference,while there was no significant difference between nurses,behavior score and patients,satisfaction score.Conclusions1.Hemodialysis nurses showed a fairly strong awareness of privacy protection in patients,especially in nurses who are younger,lack of administration positions,owning training experience and a better understanding of relevant laws and regulations.However,due to the incomplete department allocation and insufficient human resources of nurses,there was a bad implementation of privacy protection for patients.This suggests that we should translate nurses,cognition into action to improve the quality about privacy protection in patients'2.It was unnecessary for hemodialysis patients about privacy protection.However,we should pay more attention to the patients who are younger and religious people.Meanwhile,hemodialysis patients were not satisfied with nurses in the aspect of privacy protection.3.In our study,we found that hemodialysis nurse's perception was stronger than patients needs,which was helpful for privacy protection work.The poor self-evaluation and satisfaction about nurses5 demostrated that the actual behavior of privacy protection in patients'needs to be continually improved and consummated.
