| Purpose:Analysis the microecological diversity of cervical cancer,HSIL and normal cervical tissue,to explore the clinical significance of cervical tissue flora and cervical lesions.Provide new ideas and research directions for clinical prevention and treatment of cervical cancer.To observe the clinical efficacy of oral administration and external treatment of TCM use in the prevention and treatment of cervical intraepithelial neoplasia,and to explore the correlation between TCM on the development of cervical microecology,HPV infection and cervical lesions.Give full play to the distinctive traits of TCM to intervene in the occurrence and development of cervical intraepithelial neoplasia.To achieve the goal of preventing and treating cervical cancer.To guide the development and research of new clinical drugs.Methods:Cervical cancer,cervical high-grade intraepithelial neoplasia and norm al cervical tissue were analyzed in 5 cases(both cervical cancer and HSIL patients were infected by HP-HPV)for cervical micro-ecological diversity analysis.To understand the correlation between cervical flora and cervical lesions.And,60 cases of gynecological patients from Jiangsu Provincial Hospital of Traditional Chinese Medicine from May 2018 to January 2019 were selected,who were diagnosed as cervical intraepithelial neoplasia associated with high-risk HPV infection,and TCM syndrome type is damp heat accumulation certificate.According to the degree of lesions they were divided into two parts:HSIL group and LSIL group.Each of them were randomly divided into Chinese medicine group(oral administration and external treatment of TCM group)(n=15)and control group(Baofu Kangshuan group)(n=15).1 month is a course of treatment,patients needed to be treated for 3 courses.Evaluate the curative effect by comparing the HPV conversion rate,TCM syndrome scores,and clinical symptom scores before and after treatment.Combine previous research results,we want to observe the correlation between Chinese medicine and the development of cervical-vaginal microecology,HPV infection and cervical lesions.Results:1.Micro-ecological analysis of cervical tissue:The total amount of cervical tissue species increased with the increase of sample size,and the amount of core species decreased with the increase of sample size.As the degree of cervical lesions increases,the richness and diversity of cervical tissue microorganisms gradually increase.2.Cervical tissue flora composition:From the level of the door,the cervical flora is mainly composed of four doors:Firmicutes、Acitinobacteria、Fusobacteria、Proteobacteria.This is consistent with the composition of the vaginal microenvironment community.At the subordinate level,there are many kinds of cervical tissue bacteria.The richness of the species of the genus is positively correlated with the degree of cervical lesions.In pathological cervical tissue the abundance of conditional pathogens is higher,such as:Sneathia、Prevotella、Atopobium、Gardnerella、Providencia、Lysinibacillus、Morganella.etl.The normal cervical tissue is relatively high in abundance of Lactobacillus,and Lactobacillus is also a genus of vaginal bacteria.3.Oral administration and external treatment of TCM has a certain regulatory effect on the cervical-vaginal microenvironment,which can increase the body’s clearance rate of HPV and reduce the body’s susceptibility to HPV by interfering with the recovery of its micro-environmental stability,and intervene in the occurrence and development of cervical diseases.4.Negative conversion rate on HPV:In LSIL group,there were eleven people turned negative in Chinese medicine group(the negative conversion rate was 73.33%),control group had nine people turned negative(the negative conversion rate was 60.00%).In HSIL group,there were twelve people turned negative in Chinese medicine group(the negative conversion rate was 80.00%),control group had ten people turned negative(the negative conversion rate was 66.67%).Both of them were no statistical difference,but the conversion rate of the Chinese medicine group was higher than that of the control group.5.Comparison of TCM syndrome scores and clinical symptom scores:The TCM syndrome scores and clinical symptom scores of the LSIL group and the HSIL group were significantly decreased before and after treatment.6.Treatment efficiency:The total effective rate of the LSIL group and the HSIL group was higher than that of the control group,and the number of cures was higher than that of the control group.Conclusion:The Chinese medicine group and the control group can significantly improve the TCM syndrome and relieve the clinical symptoms in the treatment of cervical intraepithelial neoplasia.Although there was no statistical difference between the Chinese medicine group and the control group in the HPV conversion rate and cure rate,the number of patients in the cervical epithelial neoplasia group,the HPV conversion rate,the number of cures,and the total effective rate of treatment were all higher than the control group.It shows that Chinese medicine has research value in the prevention and treatment of cervical intraepithelial neoplasia,which is worthy of further study in clinical practice.The diversity of cervical tissue community and the abundance of pathogenic flora are increased with the severity of the lesion,and pathogenic bacteria gradually replace dominant bacteria and cause disease.Complex cervical microenvironment is associated with persistent HPV infection and development of cervical lesions.TCM can regulate cervical-vaginal micro-ecology,increase the body’s clearance of HPV,reduce the body’s susceptibility to HPV,and thus interfere with the occurrence and development of cervical lesions.That also provides new clinical ideas for Chinese medicine to prevent and treat cervical cancer from a microscopic perspective. |
