| Objective: To investigate the current status of continuation care services for breast cancer patients and the symptoms burden of breast cancer patients at different stages,the characteristics of continuation of nursing needs and the status of quality of life;summarizing advanced experience at home and abroad,based on field research,combined with relevant theoretical framework,based on new media to build breast cancer discharge The patient's health management platform;trial run the platform,evaluate its effects and further improve the platform operation.Methods: Part ?: Current status of continuing care needs for breast cancer patients A convenient sampling method was used to select breast cancer patients from breast surgery in a top three hospital in Changchun City from November 2017 to March 2018 to conduct a questionnaire survey to understand the nursing needs of breast cancer patients at different stages.The survey tool used: General Information Questionnaire,breast cancer patients continue to care service status questionnaire,Anderson Symptom Inventory scale,Function Assessment of Cancer Therapy-Breast Quality of Life Instrument scale,breast cancer patients continued care needs scale.Descriptive analysis of general information on breast cancer patients,status of extended care services for breast cancer patients,recommendations for breast cancer continuation care,quality of life of breast cancer patients,symptom burden of breast cancer patients,and continual care needs of breast cancer patients,consistent with normal distribution Measurement data are expressed as mean ± SD.Pearson correlation analysis was used to explore the correlation between quality of life,symptom burden and continuing care needs of breast cancer patients.To analyze the influencing factors of continuation nursing care needs of breast cancer patients: the measurement data that accords with normal distribution and homogeneity of variance in single factor analysis adopts two independent sample t-test and variance analysis,which does not conform to normal distribution and/or homogeneity of variance.Measurement data were analyzed by non-parametric test;multiple linear regression analysis was used to analyze the influence relationship between various factors and its impact on the continuing care needs of breast cancer patients.Part II: Construction and trial operation of breast cancer health management platform According to the previous research results,the breast cancer health management platform was constructed,and the professional technicians completed the design,development and release of We Chat public number and small program.40 patients were selected for platform function test to understand the platform use effect.The research tools of this research include general information questionnaire,We Chat public number user experience questionnaire.Descriptive analysis of general information on breast cancer patients and user experience of We Chat public number,and measurement data in accordance with normal distribution are expressed as mean ± SD.Understand the patient's evaluation of the public's tweet quality,page layout and etc.Results:Part ?: Current status of continuing care needs for breast cancer patients 1.Breast cancer patients' continuation nursing needs scale has 5 dimensions,42 items,Cronbach's alpha coefficient is 0.939,KMO value is 0.865,indicating that the scale has good reliability and validity.2.A total of 226 valid questionnaires were distributed.The survey showed that most patients were satisfied with the continuing care service,with 92 patients(95.84%).It is hoped that the continuation of nursing staff will be based on doctors,professional follow-up personnel,and breast nurses.3.Breast cancer patients are at a mild to moderate symptom burden level,and the symptom burden items are divided into(3.78±1.41)points.The quality of life of breast cancer patients was low,and the quality of life was divided into(82.86±18.30)points.4.The differences in age,nationality,culture level,working status,occupation,average monthly household income,type of medical insurance,and breast cancer patients' continuing care needs were statistically significant(P<0.05).5.Breast cancer patients had different levels of understanding of continuation care,and patients had different levels of acceptance of continuation care.The difference in continuation of nursing needs was statistically significant(P<0.05).The physiological status,additional attention,and total quality of life of breast cancer patients were negatively correlated with the continuous nursing needs of patients.The social family status was positively correlated with the continuous nursing needs of breast cancer patients,and the difference was statistically significant(P<0.05).7.The severity of symptoms,symptoms and symptoms of breast cancer were positively correlated with the need of continuing care for breast cancer patients,and the difference was statistically significant(P<0.05).The monthly income level of breast cancer patients and the willingness to receive continuation care have a positive predictive effect on the total score of continuation nursing needs.The age of patients has a negative predictive effect on the total score of continuing care needs,and the difference is statistically significant(P<0.05).Part II: Construction and trial operation of breast cancer health management platform Forty patients with breast cancer in this study completed the trial of the public number.The experience of breast cancer patients on the ?Pink Communication Platform? is divided into(4.13±0.58)points,and the total average score of each dimension item is above 4.00 points,indicating that patients have better experience of ?Pink Communication Platform?..The highest number of entries in the public function functional experience dimension is divided into ?convenient to view articles?,and the lowest is divided into ?automatic reply function?.The highest scores in the public sentiment experience dimension are classified as ?fan interaction is good? and the lowest is divided into ?stronger Interesting,the highest content of the public content content dimension is divided into "tweet content",and the lowest is divided into "tweet content can meet their own needs."Conclusions: 1.The implementation of breast cancer continuation care is better,and the forms of continuation care received by patients are more diverse.Most patients are satisfied with the continuation of care and have a higher willingness to accept them;Breast cancer patients have a mild to moderate level of symptom burden,the quality of life of breast cancer patients is also at a low level.2.Breast cancer patients have higher continuation nursing needs.Age grouping,ethnicity,cultural level,working status,occupation,average monthly income,type of medical insurance,place of residence,and understanding of breast cancer among patients are the continuing care needs of breast cancer patients.Influencing factors.The quality of life of breast cancer was negatively correlated with demand,and the burden of symptoms was positively correlated with demand.Age grouping,average monthly income,and the patient's willingness to receive extended care are independent factors influencing their continued care needs.3.The patient has a good experience in using the ?Pink Communication Platform?.The dimension with the highest score is the functional experience and content experience,followed by the emotional experience and user experience.Among the item scores,the patient's tweet content of the public number and the convenience of checking the article are evaluated as the highest score,and the interest of the public number and the evaluation of the public number satisfying the demand are the lowest scores. |
