Human errors in the field of medical and healthcare should not be underestimated.The consequences are accidents,doctor-patient disputes and even personal injury of patients.Therefore,it needs the study of human error in the field of medical and healthcare,observation of its performance,analysis of its causes and summary of its laws,and on this basis,improve the prevention and treatment of human error and the ability of medical activity system to accommodate human errors,so it has an urgent practical value to improve the operation quality of doctors and nurses and the personal safety of patients.On the basis of reading a large number of related theories and papers,human error identification methods,ergonomics,cognitive psychology and other theories,this paper establishes a human error identification method for ophthalmic surgery,which is suitable for ophthalmic surgery to realize human error identification.This method is divided into three steps: firstly,the External Error Mode of Ophthalmic surgery is identified,and the traditional External Error Mode is integrated to find the errors of behavioral level.At the same time,this method establishes the economic principle of motion suitable for surgery to solve the problem that skilled operators will make mistakes,and makes the economic principle of motion as follows.In order to investigate errors of behavioral level,the traditional External Error Mode is combined,so as to find out the errors of behavioral level more comprehensively.The second step is to find,identify and explore the Internal Error Mode of psychological cognitive process,and divide the human mind into four dimensions: omission,quality,time and sequence.This method establishes Internal Error Mode classification framework to find the Internal Error Mode corresponding to External Error Mode,and adds the analysis of cognitive process to the research of eye movement measurement in the field of perception.By analyzing the difference of cognitive process reflected by the difference of eye movement indicators between novice doctors and skilled doctors with mature cognitive process,the reason of that novice doctors are prone to make errors is preliminarily explored.Last but not least,searching for the Performance Shaping Factor in ophthalmic surgery,this method establishes a classification framework of Performance Shaping Factor in ophthalmic surgery environment.By analyzing the impact of environment factors on human cognitive function,it can reveal the Performance Shaping Factor that may affect human errors in ophthalmic surgery and its qualitative relationship with Internal Error Mode,so as to determine the root causes of human errors.Finally,taking the classical operation of endoscopic dacryocystorhinostomy in ophthalmic surgery as an example,the application of this method is studied.The action and operation process of surgeon are taken as the research object,and the method is used to identify and analyze the human errors of the surgery.The results show the method can effectively detect the human error modes and causes in ophthalmic surgery,thus laying a foundation for the prevention and treatment of human error,and providing reference guidance for medical staff.Based on the traditional human error identification method,this study proposes a human error identification method suitable for ophthalmic surgery to explore the human reliability of surgery.This method can be moderately adjusted and extended in more surgeries in the field of healthcare.It is of great significance to improve the quality of doctors' operation,guidance training and patient safety.
