PurposeThe diagnostic efficacy of high resolution T2-weighted magnetic resonance imaging?HRT2WI?in extramural vascular invasion?EMVI?of rectal cancer was determined by systematic analysis of meta-analysis,To explore its clinical application value and provide basis for clinical decision-making.Materials and methodsSearching articles related to high resolution T2-weighted magnetic resonance imaging,rectal cancer and extramural vascular invasion in CNKI?Wanfang data?VIP and PubMed?EMBASE?Cochrane Library,search the time is from January 2008 to October 2018.After screening the corresponding documents according to the established inclusion and exclusion criteria,the quality of the literature was evaluated according to the corresponding items of QUADAS-2?Quality Assessment of Diagnostic Accuracy Studies-2?.Subsequently,we extract the data from the literature,and we need to be able to calculate the data of four tables,and construct a 2*2 contingency table containing true positive value?TP?,false positive value?FP?,false negative value?FN?and true negative value?TN?.Statistical analysis and related chart production were performed using Stata 15.1 and RevMan 5.3 software.The heterogeneity test was performed by chi-square test,According to Meta analysis of diagnostic experiments,a bivariate model was used,The diagnostic performance of all the studies was quantitatively combined,including combination sensitivity?SEN?,specificity?SPE?,negative-positive likelihood ratio?NLR,PLR?,diagnostic ratio?DOR?.The SROC was drawn and the area under the curve?AUC?was calculated.Finally,the publication bias was expressed by funnel plot.ResultAfter screening,a total of 7 studies met the inclusion criteria,3 in Chinese and 4 in English.Among them,215 of 748 cases had extra-vessel vascular invasion,and the positive rate was 0.29.RevMan 5.3 software was used to evaluate the quality of the literature,The results showed that the quality of the seven articles included was high.The chi-square test was used to test the heterogeneity of the literature.The heterogeneity was expressed by I2.The heterogeneity test results showed that Q=2.148,df=2.00,p=0.171,I2=7,95%CI was?0-100?,indicating that the study has no significant heterogeneity.The combined diagnostic efficacy of all studies showed that the sensitivity was 0.69?95%CI0.61-0.76?,specificity was 0.90?95%CI0.84-0.94?,positive likelihood ratio was 6.90?95%CI4.0-11.7?,negative likelihood ratio was 0.35?95%CI0.27-0.45?,diagnostic ratio was 20?95%CI10-39?,and the area under the SROC curve was AUC=0.80?95%CI 077-0.84?.ConclusionHigh resolution T2-weighted magnetic resonance imaging has high specificity and moderate sensitivity for extramural vascular invasion of rectal cancer.It can accurately diagnose extramural vascular invasion of rectal cancer,and is worthy of clinical application and promotion.
