Objective:Through retrospective analysis of gynecological emergency case data in the past6 years,to understand the types of gynecological emergency,the trend of the disease,the characteristics of the disease,to explore the diagnosis and treatment of gynecological emergency,to provide clinical information for young doctors,improve the diagnosis and treatment rate of emergency patients,reduce Clinical misdiagnosis and missed diagnosis rate.Methods:The clinical data of 2455 patients in emergency gynecological hospital of the First Hospital of Jilin University from January 1,2012 to December 31,2017 were retrospectively analyzed.Statistical methods:Statistical analysis was performed using SPSS 23.0.Descriptive statistics were used for clinical data.Counting data were expressed as(%),comparative data were analyzed by X~2 test,and measurement data were analyzed by nonparametric test.P<0.05 was statistically significant.Results:1.The number of gynecological emergency patients admitted from December2012 to December 2017 was:310 cases(accounting for 12.63%),368 cases(accounting for 14.99%),414 cases(accounting for 16.86%),467 cases(accounting for 19.02%),442 cases(accounting for 18.00%),454 cases(accounting for 18.49%).The number of gynaecological emergency cases is on the rise.2.The hospitalization time of emergency patients is counted by quarter,660 cases were in the first quarter(Accounting for 26.88%),629 cases in the second quarter(accounting for 25.62%),588 cases in the third quarter(accounting for 23.95%),578 cases in the fourth(accounting for 23.54%).Emergency gynecologic care usually occurs in the first half of the year,the first quarter is the peak period of onset.3.Among the 2455 patients,the youngest was 2 years old,the oldest was 83years old,and the average age was 31.13±9.753 years old.The age of onset of the data was grouped by age group of 10 years old,including 29 cases in the age group less than 10 years old(accounting for 1.18%),164 cases in the 10-19 age group(accounting for 6.68%),985 cases in the 20-29 age group(accounting for40.12%),842 cases in the 30-39 age group(accounting for 34.30%),344 cases in the40-49 age group(accounting for 14.01%),67 cases in the 50-59 age group(accounting for 2.73%),16 cases in the 60-69 age group(accounting for 0.65%),5 cases in the70-79 age group(accounting for 0.20%),3 cases in the 80-89 age group(accounting for 0.12%).It can be seen that gynecological emergency patients are mainly concentrated in the childbearing age of 20 To 39 years old(accounting for 87.42%),and the peak age of onset is from 20 to 29 years old.4.2455 cases of gynecological emergency patients by disease:1496 cases of pregnancy-related diseases,accounting for 60.94%of gynecological emergency,for the most common type of gynecological emergency;Including 1339 cases of ectopic pregnancy(including 76 cases of cesarean scar pregnancy),accounting for 89.50%:117 cases of hemorrhagic shock.Abortion occurred in 146 cases,accounting for9.76%,of which 3 cases were complicated by hemorrhagic shock.7 cases of hydatidiform mole,accounting for 0.47%;3 cases of embryo residual,accounting for0.2%;1 case of late postpartum hemorrhage with hemorrhagic shock,accounting for0.2%.It can be seen that ectopic pregnancy is the most common pregnancy-related gynecological emergency.5.The age of onset of pregnancy-related diseases is mainly concentrated in 20-39years old(accounting for 87.44%),on-pregnancy-related bleeding disorders are most common in women aged 20-29(accounting for 44.19%),the age of traumatic disease and vaginal foreign body is concentrated in women aged 0-29(accounting for82.55%),The age of onset of infectious diseases is 10-69 years old,of which 73 cases are 40-49 years old(accounting for 42.20%),which is a high-risk age group,tumor diseases can be seen in all ages,with a large age span,most commonly females aged20-29(accounting for 29.07%);other types of diseases have no obvious age characteristics.It can be seen that there are significant differences in the distribution of different ages of gynecological emergency diseases.6.Among 2,455 gynaecological emergency patients,1815 have been born,and640 have not been fertile.Among 1815 fertile patients,1219 cases of pregnancy-related diseases(accounting for 67.16%),232 cases of non-pregnancy-related bleeding disorders(accounting for 23.44%),179 cases were tumor diseases(accounting for 9.86%),147 cases were acute infectious diseases(accounting for8.10%),16 cases of traumatic diseases and vaginal foreign bodies(accounting for0.88%);among 640 non-fertility gynaecological emergency patients,277 cases were pregnancy-related diseases(accounting for 43.28%),150 cases of non-pregnancy-related bleeding diseases(accounting for 23.44%),110 cases of neoplastic diseases(accounting for 17.19%),70 cases of traumatic diseases and vaginal foreign bodies(accounting for 10.94%),26 cases of acute infectious diseases(accounting for 4.06%).7.Among 2455 gynecological patients,1033 urban women and 1422 rural women,there are more rural patients than cities.Among 1033 urban gynecological emergency patients,the proportion of non-pregnancy-related hemorrhagic diseases,neoplastic diseases,traumatic diseases and vaginal foreign bodies was higher than that of rural women;1422 rural patients were significantly higher in pregnancy-related diseases and infectious diseases than urban women.The difference was statistically significant.Conclusion:1.In recent years,the overall gynecological emergency patients have shown an upward trend,which occurs in first half of the year,the first quarter is the fastigium.2,Gynecological emergency patients are mainly concentrated in women in the reproductive period,and most of them are rural patients.3,The characteristics of gynecological emergency diseases in different age groups are different.The vaginal foreign body is mainly in 0-9 years old,the non-pregnancy-related bleeding diseases are mainly in 10-19 years old,and the pregnancy-related diseases are mainly in 20-49 years old.Mainly tumoric diseases.4,Ectopic pregnancy is the most common gynecological emergency,and more emergency surgery is needed.
