Objective:To observe the effect of ultrasound-guided posterior quadrates lumbar block combined with intravenous analgesia pump on analgesia after cesarean section.Methods:We designed a randomized controlled trial to observe the effect of ultrasound-guided posterior quadrates lumbar block on postoperative analgesia after cesarean section.60 patients who underwent cesarean section in our hospital from June 2017 to December 2017 were enrolled.All women were randomly divided into two groups: posterior quadrates lumbar block(Q group)and control group(C).The VAS scores,uterine contraction pain score,Ramsay sedation score,BCS comfort score were observed at 2 h,4 h,8 h,12 h,and 24 h after surgery.The number of postoperative PCIA compressions in 24 hours and PCIA-related adverse reactions such as constipation,nausea and vomiting,urine retention,rash,and respiratory depression were recorded.The adverse reactions such as hematoma and infection at the puncture site were recorded;the effects of pain on the patients at each time point were recorded.Results:The VAS scores of the Q group were significantly lower than those of the C group at 2h,4h,8h,12 h and 24 h after operation(P<0.05).The uterine contraction pain scores of the Q group were lower than those of the C group at4 h,8h,12 h and 24 h after operation(P<0.05).The BCS comfort scores of group Q were higher than those of group C at 4h,8h,12 h and 24 h after operation.The number of postoperative PCIA compressions in group Q was less than that in group C.There was 1 case of nausea and vomiting in the Q group,and 4 cases of nausea and vomiting occurred in the group C.There were no other adverse events associated with PCIA in the two groups of women,and there was no difference in the Ramsay sedation scores between the two groups.Conclusions:Postoperative cesarean section with ultrasound-guided posterior lumbar muscle block combined with PCIA postoperative analgesia can reduce postoperative VAS score and uterine contraction pain score,improve BCS comfort score,reduce postoperative PCIA compressions in 24 hours,and reduce the amount of tramado.
