| ObjectiveThe clinical manifestations,MRI manifestations,surgical approach,postoperative complications and prognosis of central neurocytoma were analyzed,so as to further understand central neurocytoma and improve the diagnosis and treatment level of this disease.Methods45 cases of central neurocytoma confirmed by microsurgery in the department of neurosurgery of the first affiliated hospital of zhengzhou university from June 2013 to May 2018 were selected and analyzed from the aspects of clinical manifestations,imaging examination,surgical approach,pathological results and prognosis combined with domestic and foreign literatures.GTR Gross Total resection 全部切除STR Subtotal resection 次切除PR Partial resection 部分切除ResultAll 45 patients underwent surgical treatment,microsurgical gross total resection 25 cases(55.6%),subtotal resection 15(33.3%),and partial resection in 4 cases(8.9%),biopsy 1 case(2.2%),according to the result of pathology for radiation therapy,radiation therapy period generally choose postoperative 1 month or so,dose of 45-60 gy,some cases give alkylating agent drugs for mo thiazole amine capsules or nitroso urea class f SiTing.Except 1 patient 6 hours after sudden death,more than 44 cases of postoperative cases were followed up for 6 ~ 5 months,3 cases(6.8%)recurrence and severe hydrocephalus in 3 patients(6.8%),the time of postoperative respectively for 30 days,37 days,17 months,all lines of ventricle peritoneal shunt,1 patient presented with intraoperative cyst and underwent cyst ventriculocephaly.,1 case of postoperative hypothyroidism,1 patient had deafness in both ears,2 patients had memory loss,and 2 patients had mutism.6 patients showed motor dysfunction: among them,2 patients had severe hemiplegia,which affected their normal life;3 patients had mild muscle strength decline in one limb;1 patient had difficulty holding only the left upper limb.ConclusionCentral neurocytoma is mostly located in lateral ventricle,which has a high risk of operation and a high mortality and disability rate.Microneurosurgery,intraoperative navigation and other techniques can effectively reduce the mortality and disability rate after operation.Gross total resection is the best way to treat CNC,and the prognosis of total tumor resection is good,which can reduce the recurrence rate to the minimum.Postoperative radiotherapy can reduce the recurrence. |
