| Objective To investigate the predictive value of p53,Ki67,p14 ARF,E-cad,C-myc,Ras and EGFR in regional lymph node metastasis(RLN M)and prognosis in patients with penile cancer.Methods Specimens of 41 patients with surgically treated penile canc er were collected and fabricated into tissue of penile cancer tissue using t issue chip technology.Immunohistochemical staining was used to detect th e expression of seven molecular markers of p53,Ki67,p14 ARF,E-cad,C-myc,Ras and EGFR in cancer tissues,paracancerous tissues and positive lymph nodes of 41 patients with penile cancer.Fisher’s exact test,Logistic regression analysis,Kaplan-Meier survival curve and other statistical meth ods were used to analyze the relationship between p53,Ki67,p14 ARF,E-cad,C-myc,Ras,EGFR expression and RLNM and prognosis of penile cancer.Result 1.The positive rates of p53,p14 ARF,E-cad,C-myc,Ras and EGFR in the cancer tissue of penile cancer patients are 55.6%、96.3%、85.2%、96.3%、63% and 51.9%,their positive rate are significantly highe r than that of the paracancerous group,and the difference was statistically significant(P<0.05).The positive expression rates of p53、Ki67、p14AR F、E-cad、C-myc、Ras and EGFR in the positive lymph node group are 50%、12.5%、87.5%、62.5%、85.7%、85.7% and 85.7%,the positive rat e of p53、E-cad、Ras、EGFR are significantly higher than that of the pa racancerous group(P<0.05),but Ki67、p14ARF、C-myc are no significant difference in the positive rate of myc between the positive lymph node group and the paracancerous group(P>0.05).There are no significant diff erence in the positive rates of p53、Ki67、p14ARF、E-cad、C-myc、Ras and EGFR between the penile cancer group and the positive lymph node group(P>0.05).2.The expression of p53、Ki67 and EGFR in penile cancer tissues was positively correlated with RLNM in penile carcinoma(P<0.05).Ag e、T stage、M stage、pathological grade、p14ARF、E-cad、C-myc and R as were not associated with RLNM in penile cancer(P>0.05).3.Multivariate analysis showed that p53 was an independent risk fact or for RLNM in penile cancer [P=0.038,OR(95% CI)=18.238(1.178~282.372)].4.Combined multi-indicator diagnostic model(In[p/(1-p)]=2.904×p53+21.048×ki67+1.882×E-cad-3.334)of p53、Ki67、EGFR for predicting the sensitivity of RLNM in penile cancer is 88.9%,specificity is 77.8%,Posit ive Predictive Valueis 66.7%,negative predictive value is 93.3%,under R OC curve Area(AUC)= 0.906.5.The expressions of p53、Ki67、p14ARF、E-cad、C-myc、Ras and EGFR in penile cancer tissues were not significantly correlated with T st age、M stage、pathological grade and age(P>0.05).6.The average follow-up time was 83 months.The overall 5-year ca ncer-specific survival rate was 66.7%(18/27).Kaplan-Meier analysis revea led p53 in penile cancer.The expression of the 5-year CSS associated wi th penile cancer patients(P=0.025): p53 expression positive rate increased,5 years CSS decreased.Other indicators were not significantly associated with penile cancer 5 years CSS(P>0.05).Conclusion 1.Increased expression of p53,E-cad,p14 ARF,C-myc,Ras,EGFR may be associated with the development of penile cancer.2.Increased expression of p53 has a certain predictive effect on lym ph node metastasis in penile cancer.3.Combined detection of multivariate diagnostic models of p53,Ki67,and EGFR can improve the accuracy of predicting lymph node metasta sis in penile cancer.4.Increased expression of p53 is associated with poor prognosis of p enile cancer. |
