BackgroundInfectious diseases are rising every year,and remain the leading cause of death in developing countries.Clinical microbiological examination has played an important role in the prevention and treatment of infectious diseases.With the continuous deepening of China's medical and health system reform,and the actual developed needs of clinical microbiology testing disciplines,the voices of all parties to improve the unreasonable price are increasing.In order to reflect the principle of evidence-based decision and explore the real operating cost of microbial testing technology in the real environment,this study used blood culture testing technology as the point,carried out full cost estimation and analysis,it helps to improve the accuracy of cost accounting and the competition of hospitalsPurposesThrough qualitative interviews,we can understand the development status of microbiology testing disciplines,technical pricing appeals,staffing situation and clinical practice challenges;Quantitative analyzed the cost structure of blood culture in medical institutions,and compared costs with current charges;Therefore,we can find out the existing problems in the development of clinical microbiology testing in China and put forward suggestions in line with national conditions.MethodsIn this study,13 hospitals were selected as the object in 7 provinces in China.The research methods were applied to the study:activity-based costing,expert consultation and on-the-spot investigation.It calculated the full cost of blood culture test and analyzed the cost structure of blood culture in medical institutions of various provinces.The qualitative analysis was mainly carried out in the form of semi-structured group interviews.We interviewed with the main stakeholders of hospitals,transcribed and analyzed the interview data.Finally,we summarized the microbiological examination discipline,talent retention and pricing.Research resultsThe results of the quantitative analysis showed that the cost of blood culture bottles was higher in the blood culture,regardless of the tertiary or the secondary medical institution.The identification accounted for about 20%of the total cost in tertiary and secondary medical institutions,and the proportion of labor costs in microbial testing was small;the cost of blood culture and identification of 13 medical institutions was generally higher than the standard of charge.In the actual work,the insufficient attention to microbiology laboratory affects the effectiveness of microbial testing.The human resources of the microbiology laboratory were insufficient,and contact between clinician and inspector was rare.Policy recommendationsIt clarified the cost structure of blood culture and identification,and help to detail and control cost of the laboratory;It formulated the appropriate pricing mechanism and charging standard,which based on the actual cost consumption and standardized operation process;On the premise of subject development,we should pay more attention and support to the subject of microbiological testing in hospitals;We can improve specialization and cooperation of microbiological testing according to the actual clinical needs.
