Background: The rapid on-site evaluation(ROSE)technology is a rapid interpretation techniques,which is associated with diagnostic endoscopic intervention operation;it calls the relevant inspection personnels and the needed operation premise prepared and collects specimens by methods such as biopsy,brush,lavage or puncture to make smearing and staining for the obtained specimens quickly;and timely uses microscope to rapidly evaluate the qualified satisfaction of specimens,classify the specimens and then make an preliminary diagnosis in order to indicate the next work.The English word “rose” also stands for a flower that called “Meigui” in Chinese,so we also call ROSE technology as “Meigui” technology.ROSE consists of two major parts: cytological rapid on-site evaluaiton(C-ROSE)and microbiological rapid on-site evaluation(M-ROSE).In recent years,the incidence of lung cancer and the infection rate of drug-resistant bacteria in the lower respiratory tract have increased significantly,and the urgent need for the diagnosis and treatment in difficult and critical respiratory diseases,has promoted the development of diagnosis endoscopic intervention in pulmonary in various degrees.In lung epidemiology endoscopic diagnostic intervention operations,C-ROSE technique is in a auxiliary status.Master its main points of cytological interpretation,use its real-time accompanying advantage fully and reasonably,acquire the target sampling materials successfully,make cytological slides rapidly under the basic premise of not damaging tissue samples,give dyeing,and then use microscope to interpretate with intergrated clinical information.Its interpretations consists of cell morphology,cell classification,cell composition ratio ? permutation and correlation,background and exotic object analysis in the field of vision.Same with the pahtology and laboratory medicine,ROSE technique is also one of the cytology carriers.So it has functions that not only make the the interventional physicians performe the pathogenetic analysis and outcome anticipation combined with all clinical information and cytology condtion but also evaluate the satisfaction degree of the sampling,real-time guidance intervention operationmeans and ways,optimizing the further processing scheme of target specimens and narrowing the scope of differential diagnosis.The specific operations of C-ROSE are divided into three steps: making specimen slides,staining specimen slides and interpreting slide results;and its technical core lies in two points that increasing the speed of making specimen slides and dyeing as much as possible,interpreting the cytological results timely and accurately.Objective: To explore and analyze the application results of cytological rapid on-site evaluaiton(C-ROSE)technique in the interventional diagnosis of lung space-occupying disesses within the reach of fiberoptic bronchoscopy.Compare and analyze the postive results of conventional cytological group and C-ROSE group with the consistency of the final pathological results respectively so as to evaluate its clinical application value.Methods: Collect and Analyze the 106 patients who underwent the fiberoptic bronchoscopy and met the inclusion criterias from September 1,2017 to March 31,2019 in the Interventional Endoscopy Room of the Respiratory Department in Jinan Central Hospital affiliated to Shandong University.All patients were the lung space-occupying lesions within the accessible range under the fiberoptic bronchoscopy,including 53 cases in the C-ROSE group and 53 cases in the conventional cytological group.The final pathological results of all the selected cases were tracked;statistical methods were used to analyze the consistency of two groups,results and their respective consistency with the final pathological results so as to draw a conclusion.Results:(1)The first definite diagnosis ratio of the C-ROSE group and the conventional cytological group was 73.58%?54.72%,respectively.The coincidence rato compared with the pathological results was 86.79%?73.58%,respectively.The coincidence ratio of Kappa in C-ROSE group was 0.724 and was 0.456 in the conventional cytological group,indcating that the coincidence ratio in the C-ROSE group was higher than the conventional cytological group.However,there was no difference in the diagnosis ratio between the two groups(p>0.05).(2)The average nubmer of biopsies the C-ROSE group and the conventional cytological group was 4.91 ? 8.47,respectively,and the difference was statistically signnificant(p <0.0001).(3)The incidence ratio of the second examiniation due to the failure in the first diagnosis of the C-ROSE group and the conventional cytological group was 11.32%?28.30%,respectively,and the difference was statistically signnificant(p<0.05);(4)The moderate tomassive bleeding incidence of the the C-ROSE group and the conventional cytological group during biopsy was 9.43% ? 13.21%,respectively,and the difference was not statistically signnificant(p>0.05).Conclusion: For the patients who have suffered from lung space-occupying diaeases that was diagnosed by the chest CT or PET-CT and within the reach of fiberoptic bronchoscopy,compared with the conventional cytological examination,fiberoptic bronchoscopy combined with cytological rapid on-site evaluaiton(C-ROSE)technique has:(1)the coincidencce rate in line with the results of final pathology diagnosis was increased significantly;(2)it reduces the operating times of necessary invasive examination,reduces the blindness in the operating process,so reduces the pain of patients;(3)it greatly improves the work efficiency of clinical respiratory interventional physicians and promotes the further development of clinical respiratory center;(4)we believe that on the basis of the applying development of cytological rapid on-site evaluaiton(C-ROSE)technique,in addition to the needs of modern clinical diagnosis and treatment,it is worth nothing that the development prospect in other fields related the respiratory ROSE technology.
