Observation The Clinical Effect Of VSD On Refractory Wounds Under Different Negative Pressure Range

Objectives:The closed negative pressure drainage technology(vacuum sealing drainage,the VSD)under different negative pressure applied to the hand and foot painless after wound,by observing the fresh wound granulation tissue coverage and tube plugging rate differences,explore the hand and foot painless under different negative pressure wound of the clinical curative effect,so as to obtain the hand and foot painless wound optimal pressure range when the VSD treatment technology,and discuss the main reasons.Methods:78 cases(39 cases)of hands were selected from the affiliated hospital of hebei university of technology from September 2016 to October 2018.All the 78 patients were treated with VSD technology,and the clinical efficacy(growth of granulation tissue and blockage rate)was analyzed under different negative pressure.The patients were randomly divided into six groups according to the location of refractory wounds and the order of admission,including three groups of hand wounds and three groups of foot wounds,with 13 cases in each group.There were three groups of refractory hand wounds(A1,A2 and A3)and three groups of refractory foot wounds(B1,B2 and B3).The negative pressure range of hand wound was set as follows:A1:200-250mmhg;A2:250-300mmhg;A3:300-350 mmhg;The negative pressure range set for the foot wound surface was: B1:350-400mmhg;B2:400-450mmhg;B3:450-500 mmhg.Clinical data of 6 group were 7 days after treated with VSD techniques,comparing the different negative pressure within the scope of painless wound granulation tissue cover and tube plugging rate situation,through the analysis and processing data,observation of hand,foot wound under different pressure of clinical curative effect,according to the fresh wound granulation tissue coverage and tube plugging rate,to obtain the best negative pressure range.Results:After 7 days of VSD treatment,9 cases of fresh granulation tissue coverage were excellent,4 cases were good and 4 cases were blocked.Group A2: there were 13 cases with granulation tissue coverage and no blockage.Group A3: granulation tissue coverage was excellent in 10 cases,good in 3 cases,and blocked in 3 cases.To sum up,the observation indexes of group A showed that the negative pressure of group A2(A2:250-300mmhg)was the best negative pressure range of hand refractor wounds.After 7 days of treatment with VSD,the results showed that group B1:granulation tissue coverage was excellent in 10 cases,good in 3 cases,and blocked in 3cases.Group B2: granulation tissue coverage was excellent in 13 cases without blockage.B3 group: granulation tissue coverage was excellent in 11 cases,good in 2cases,and blocked in 2 cases.To sum up,the observation indexes of group B showed that the negative pressure of group B2(400-450mmhg)was the best negative pressure range for the foot refractory wounds.Conclusions:According to the observation and research,VSD has different clinical effects on refractory wounds under different negative pressure,and it has better clinical effects under negative pressure of 250-300 mmhg on hand refractory wounds.The foot refractory wound had a good clinical effect at 350mmHg-400 mmHg.The selection of refractory hand and foot wounds is worthy of clinical application and promotion.
