[Objective]:To explore the effects of an Internet-based remote patient care approach using a Cloud Platform established based on the self-management model for improving the level of self-efficacy in self-management,the revisit rate,and the treatment compliance rate of patients receiving targeted drug therapies for breast cancer.[Methods]:88 patients receiving targeted drug therapies for breast cancer in a top class hospital from March 2018 to December 2018 were selected and randomly assigned to the test group or control group each including 44 patients.The test group adopted a full-treatment course management approach using a Cloud Platform established based on the self-management model,where Internet-based remote dynamic management was performed by creating electronic patient records,symptoms assessment and push messages of health education,and online remote dynamic management and effectivenss evaluation were performed.The control group adopted the conventional self-management model and outpatient follow-up management.[Results]The intervention results of different self-management models for patients receiving targeted drug therapies are as follows:(1)Comparison of level of self-efficacy in self-management between the two groups:There was no significant difference between the two groups in the level of self-efficacy in self-management prior to initial use of targeted therapies(P>0.05);the level of self-efficacy in self-management was higher in the test group than in the control group at the end of the 3rd and 4th treatment cycles(P<0.05).(2)Comparison of quality of life between the two groups:There were no significant differences between the two groups in the overall quality of life prior to initial use of targeted therapies or at the end of the 3rd treatment cycle(P>0.05);the overall quality of life was better in the test group than in the control group at the end of the 6th treatment cycle(P<0.05).(3)Comparison of anxiety score between the two groups:There was no significant difference between the two groups in the anxiety score prior to initial use of targeted therapies(P>0.05);the anxiety score was lower in the test group than in the control group at the end of the 3rd and 6th treatment cycles(P<0.05).(4)Comparison of depression score between the two groups:There were no significant differences between the two groups in the depression score prior to initial use of targeted therapies or at the end of the 3rd treatment cycle(P>0.05);the anxiety score was lower in the test group than in the control group at the end of the 6th treatment cycle(P<0.05).(5)Comparison of patient revisit rate and treatment compliance rate between the two groups:The patient revisit rate and treatment compliance rate were higher in the test group than in the control group at the end of the 3rd and 6th treatment cycles(P<0.05).[Conclusion]:The Cloud Platform System features simple operations and good user friendliness,allowing patients and their relatives with different educational attainment to use independently after receiving specific instructions.The mobile end allows simple and quick completion of follow-up records,and ensures completeness of the recorded data through system alerts.The PC end for health care providers allows quick,thorough retrieval of patient follow-up data.The varying features of different client ends facilitate use by different roles,which,in addition to promoting communication between patients and health care givers,improves the effectiveness of and compliance with out-of-hospital follow-up,and reduces the loss-to-follow-up rate,which is favorable for carrying out normative follow-up remotely.The Cloud Platform collects patients'clinical data according to the characteristics of targeted drug therapies,allows collection,query and summarization of follow-up data for patients with breast cancer,which fully reflects the specialized patient condition during the treatment period and home-based rehabilitation period,and provides practical evidences for objective,in-depth evaluation of the effectiveness of this care approach.With system extensions and the technical support of big data,the features and range of clinical application of this platform can be extended,making it applicable not only to follow-up for other care approaches for breast cancer,but also to other malignancies expected with relatively long survival.Extended application range of the Cloud Platform allows continuous expansion of the medical database,which provides big data support for the development of Internet Plus healthcare,indicating its high application value.
