Objective:To investigate the survival and the prognosis of Surgical and Non-Surgical Patients with lung cancer in Xinjiang area and to explore the prognostic factors.Methods:2123 Patients with primary lung cancer admitted between January2010 and December 2016 in Affiliated Tumor Hospital of Xinjiang Medical University were selected.The surgical group and the non-surgical group were matched by the propensity scoring method(PSM)to make the groups evenly balanced.The chi-square test was used to compare the differences between the variables groups,the survival curve was plotted by Kaplan-Meier method.Log-rank tested was used for Univariate analysis,Cox regression models were used for various factors that may affect prognosis.Result:Before the study of PSM,2123 patients were included,the sex ratio was 1.85:1,the Han nationality 1830 cases accounted for 86.2%,and the average age was(62.43±1.02).Univariate and multivariate analysis showed:gender,ethnicity,age,BMI,smoking,Ventilation dysfunction,KPS,TNM stage,surgery,radio-chemotherapy,primary tumor site,site classification,presence or absence of metastasis,full-course diagnosis,CYFRA21-1 abnormality,CEA abnormality and CA125 abnormal group was the independent factor affecting the prognosis of lung cancer patients.Comparison of survival between the surgical and the non-surgical group 1532 patients with lung cancer after PSM was analyzed.The 1,3,and 5 years survival rates of the surgery group were81.2%,55.8%,and 40.5%,respectively,in the non-surgical group were 53.4%,15.6%,and 5.6%,two groups were statistically different(?~2=274.947,P<0.001).Surgical treatment improved the long-term survival rate of women,patients<60 years old,no smoking history,no history of ventilatory dysfunction,KPS>80,early TNM,peripheral type and non-metastasis.High BMI,the higher the level of education,the better the improvement on surgical treatment.Conclusion:Compared with the non-surgical group,the overall prognosis of the surgical patients is better.Male,ethnic,high age,low BMI,smoking,low KPS,high TNM stage,non-surgery and non-radio-chemotherapy,central,metastatic,CYFRA21-1 abnormality,CEA abnormality,and CA125 abnormal prognosis was relatively worse,surgical treatment The degree of improvement in their prognosis was also weaker.Active education is recommended to raising public awareness of treatment.
