Background Breast cancer is the most diagnose tumor in women in the world,the incidence of breast cancer is different in different region.There are 270,000 new breast cancer patients in China every year.Clinically,the prognosis of breast cancer is closely related to axillary lymph node metastasis.And breast cancer is a heterogeneous tumor.Clinical studies have shown that the prognosis of breast cancer is also related to various clinicopathological features.At present,the correlation between axillary lymph node metastasis and clinicopathological features of breast cancer remains controversial.This article analyzes the clinical features of axillary lymph node metastasis in breast cancer.Objective To study the relationship between axillary lymph node metastasis and clinicopathological features of breast cancer.To study the high risk factors of axillary lymph node metastasis in breast cancer,and to explain the rules of axillary lymph node metastasis in breast cancer,so as to guide the follow-up treatment and predict the prognosis of patients.Materiel and methods Patient data were collected using the method of reviewing medical records.Total of 1012 patients with invasive ductal carcinoma with complete clinical and pathological data were included,of which 422 were positive for axillary lymph node metastasis,and the positive rate of lymph node metastasis was 41.7%.Analyze the clinical and pathological data of all breast cancer routinely performed in our hospital,including patient age,histological grade(G),tumor size,estrogen receptor(ER),progesterone receptor(PR),ki-67 index,Her-2 expression,CK5/6,EGFR,molecular types.The age grouping criteria for patients were grouped using the new WHO age group.The histological grades were grouped according to the China Cancer Society guidelines and guidelines for breast cancer diagnosis(2017 edition).The tumor size was based on the T-segment standard of the Eighth Edition of the American Joint Committeeon Cancer(AJCC).ER,PR and Her-2 expression according to China guidelines,the expression level of ki-67 was grouped by 14%.The expression of EGFR and CK5/6 was confirmed by pathology,molecular types was performed using the 2017 St Gallen Expert Consensus.Chi-square test was performed using SPSS 22.0 software for differential analysis.Pearson correlation test was used to analyze the correlation between clinical features of breast cancer and axillary lymph node metastasis.Result In this article,1012 invasive ductal carcinoma patients were enrolled,including 422(41.7%)patients with positive axillary lymph node metastasis and 590(52.3%)patients with negative axillary lymph node metastasis.Chi-square test results showed that tumor size,histological grade,PR expression,ki-67 expression,Her-2 expression and molecular typing were different between axillary lymph node metastasis and non-metastasis group.But age,ER,CK5/6 and ERGR is no difference.At the same time,the correlation test showed that with the increase of histological grade,tumor enlargement,high expression of Ki-67 and overexpression of Her-2,the risk of axillary lymph node metastasis in breast cancer increased.At the same time,this article shows that there is a difference in molecular typing between lymph node metastasis and non-metastasis in breast cancer,but there is no correlation.Triple-negative breast cancer did not increase the risk of axillary lymph node metastasis in breast cancer.Conclusion Tumor size,histological grade,PR expression,ki-67 expression,Her-2 expression and molecular typing were different in the axillary lymph node metastasis group and nonmetastasis group.Increased histological grade,tumor enlargement,high expression of Ki-67,and overexpression of Her-2 increase the risk of axillary lymph node metastasis in breast cancer.At the same time,this study found that triple-negative breast cancer did not increase the risk of axillary lymph node metastasis in breast cancer.Comprehensive analysis of these factors will help clinicians make accurate choices and assessments for the treatment and prognosis of breast cancer patients.
