Background and purposeStroke has the characteristics of high morbidity,high disability and high mortality,and more than 80%of them are ischemic stroke.Due to the limitations of treatment time window and technical requirements,only a small number of people have benefited significantly.Therefore,exploring new effective treatment methods is a problem that most current cerebrovascular disease researchers are concerned about,and exploring corresponding mechanisms to find new methods is a important way.A large number of reports have reported that epigenetic changes occur in the body after ischemic stroke,such as DNA methylation,histone acetylation,miRNA changes etc.,among which the level of DNA methylation is particularly obvious.However,the current research on DNA methylation and ischemic stroke is mostly limited to the level of cells and animals Only a small part of it involves human studies.Therefore,we intend to detect peripheral blood DNA methylation-related three DNA methyltransferases(Dnmt3a,Dnmt3b and Dnmtl)and MeCP2 protein levels in patients with acute ischemic stroke and stroke screening.to explore the relationship between Dnmt3a,Dnmt3b,Dnmtl and MeCP2 and the development of ischemic stroke,in order to provides new ideas for exploring effective ischemic stroke treatment methodsInformation and methods1.Subjects:All 174 samples were from patients admitted to the Department of Neurology,Subei Hospital from March 2018 to october 2018(acute cerebral infarction group)or community stroke screening group(control group).A total of 87 patients with acute ischemic stroke(acute cerebral infarction)and 87 patients with community stroke screening(control group)were enrolled in the Department of Neurology,Subei Hospital All patients with cerebral infarction underwent MRI and recorded TOAST classification,infarct size,treatment plan,and cervical vascular ultrasound results2.Collect basic information:Collect baseline data(including age,gender,BMI,hypertension,diabetes,hyperlipidemia,smoking,alcohol consumption,coronary heart disease,atrial fibrillation,etc.)of all subj ects3.Experimental methods:The peripheral venous blood of patients with acute cerebral infarction within 24 hours of onset and the community stroke screening population were collected,and serum was stored frozen for use.ELISA was used to detect three kinds of DNA methyltransferases(Dnmtl,Dnmt3a and Dnmt3b)and protein levels of MeCP2 in acute cerebral infarction group and control group4.Statistical methods and data analysis:Statistical analysis was performed on the data using SPSS 23.0 software.The expression differences of Dnmtl,Dnmt3a,Dnmt3b and MeCP2 in the two groups were analyzed,and analyze the effect of different stroke types(grouped according to TOAST classification),carotid plaques(grouped according to cervical vascular ultrasound results),hyperacute treatment and infarct size(grouped according to imaging lesion size)on Dnmtl,Dnmt3a,Dnmt3b,and MeCP2 protein levels.Result1.There were no significant differences in age,gender,BMI,hypertension,diabetes,hyperlipidemia,smoking,alcohol consumption,and coronary heart disease between the patients with acute cerebral infarction and the control group(P values were 0.573,0.163,0.072,0.873,0.734,1,0.351,1,0.051,P>0.05)2.The levels of Dnmtl,Dnmt3a,Dnmt3b and MeCP2 in peripheral blood of acute cerebral infarction group were 761.93±313.88pg/ml,139.55±56.40pg/ml,127.44±54.78pg/ml,488.87±212.25pg/ml.The control group peripheral blood Dnmtl,Dnmt3a,Dnmt3b and MeCP2 protein levels were 637.98±182.91 pg/ml,126.75±41.43 pg/ml,105.17±40.47 pg/ml,and 422.20±172.62 pg/ml.The levels of Dnmtl,Dnmt3a,Dnmt3b and MeCP2 in peripheral blood of patients with acute cerebral infarction were higher than those of the control group.The levels of Dnmtl,Dnmt3b and MeCP2 were statistically different between the two groups(P=0.002,0.003,0.024,P<0.05),and the level of Dnmt3a was no statistically different between the two groups(P=0.09,P>0.05)3.Grouping according to the classification of TOAST,There were no significant differences in the peripheral blood Dnmtl,Dnmt3a,Dnmt3b and MeCP2 protein levels of the patients of atherosclerotic cerebral infarction group(36 cases),small artery occlusion cerebral infarction group(34 cases)and cardiogenic cerebral infarction group(9 cases)(P values were 0.544,0.632,0.141,0.654,P>0.05).There were statistics differences in Dnmt3b protein levels in peripheral blood of patients with small arterial occlusive cerebral infarction and cardiogenic cerebral infarction(P value 0.034,P<0.05)4.Grouping according to the imaging infarct area,there is no statistical differences in peripheral blood Dnmtl,Dnmt3a,Dnmt3b and MeCP2 protein levels small area cerebral infarction group(37 cases),moderate area cerebral infarction group(35 cases)and large area cerebral infarction group(13 cases)patients(P values were 0.242,0.572,0.487,0.196,P>0.05)5.Grouping according to the hyperacute treatment,the conventional treatment group(75 cases),thrombolytic group(9 cases),thrombectomy group(3 cases)peripheral blood Dnmt1,Dnmt3a,Dnmt3b and MeCP2 protein levels were not statistically significant(P The values were 0.406,0.396,0.822,0.583,P>0.05)6.Grouping according to cervical vascular ultrasound results,there were no statistical differences in peripheral blood Dnmtl,Dnmt3a,Dnmt3b and MeCP2 protein levels of patients of carotid plaque group(49 cases)and no carotid plaque group(23 cases)(P values were 0.170,0.236,0.201,0.329,P>0.05),15 cases of which do not perform cervical vascular ultrasoundConclusion1.The levels of Dnmt1,Dnmt3b and MeCP2 in peripheral blood of patients with acute cerebral infarction were significantly different from those of community stroke screening.2?The level of Dnmt3b protein in peripheral blood of patients with small arterial occlusive cerebral infarction was higher than that of patients with cardiogenic embolic cerebral infarction within 24 hours.3?Acute cerebral infarction in 24h peripheral blood Dnmtl,Dnmt3a,Dnmt3b and MeCP2 protein levels were not associated with treatment in the hyperacute phase,whether accompanied by carotid plaque and infarct size.
