Objective:To explore the application of dynamic contrast-enhanced magnetic resonance imaging?DCE-MRI?and histogram analysis in the diagnosis of alveolar echinococcosis.Methods:Ten patients with alveolar echinococcosis who met the inclusion criteria were examined by DCE-MRI with 3.0T magnetic resonance machine.Quantitative parameters of DCE-MRI including Ktrans,Kep,Ve and Vp were measured in the parenchymal area,marginal zone,edema area and corresponding contralateral normal brain parenchyma area.The histogram parameters of quantitative parameters?Ktrans,Kep,Ve,Vp?in the whole lesion area and the corresponding contralateral normal area were measured,including energy,entropy,kurtosis,skewness and 90 percentiles.SPSS19.0software was used for statistical analysis.Results:The Ktrans,Kep,Ve,and Vp values in the parenchymal lesions of cerebral echinococcosis were:0.574?0.206,1.667?,0.122?0.012,0.225?,0.469?0.145,0.600?,and 0.187?0.050,0.348?,respectively.The Ktrans,Kep,Ve and Vp values of the marginal zone are:1.477?1.066,4.113?,0.223?0.064,0.447?,0.875?0.468,1.000?,0.337?0.156,0.649?;Ktrans,Kepp in the edema area,Ve and Vp values were0.067?0.029,0.199?,0.084?0.000,0.341?,0.135?0.000,0.279?,and 0.103?0.051,0.283?.The Ktrans,Kep,Ve and Vp values of the contralateral normal brain parenchyma were respectively It is:0.008?0.001,0.044?,0.005?0.000,0.156?,0.071?0.000,0.236?,0.045?0.018,0.149?.The K-W statistics of Ktrans,Kep,Ve and Vp in the parenchymal,marginal,edema and contralateral normal parenchymal areas of cerebral echinococcosis were99.754,12.349,44.813,29.451?P<0.05?.The rank mean difference of Ktrans,Kep,Ve and Vp is significant,indicating that there is a significant difference in the overall distribution.There were significant differences in the values between the parenchymal area and the contralateral normal brain parenchyma.Compared with the edema area,the Ktransrans and Ve values were statistically significant.There were significant differences in the values of the marginal zone of the lesion compared with the edema zone and the contralateral normal brain parenchyma.The difference between Ktransand Vp was statistically significant in the lesion edema area compared with the contralateral normal brain parenchyma area.Global histogram analysis of cerebral echinococcosis lesions,entropy and 90th percentile of Ktransrans and Ve values in the parenchymal region,energy of Ktrans,Kep,Ve values,kurtosis and skewness of Ve and control zone The difference was statistically significant.Conclusion:DCE-MRI and histogram analysis provide new ideas for the diagnosis of cerebral echinococcosis and the determination of the marginal zone.
