The Effect And Mechanism Of Pelvic Integration Manipulation On Pelvic Ring Structure In Postpartum Pelvic Girdle Pain Patients

Objective:On the basis of massage and tendon therapy,then we used the pelvic integration manipulation and analysised the therapeutic effect of this techniques to patients that had the pelvic girdle pain.After that,we should analyze the clinical efficacy of pelvic plasty in the treatment of postpartum pelvic pain and observe the changes of QBPDS,JOA,ASLR scores and pelvic measurement indexes before and after treatment,which target for exploring the mechanism of pelvic plasty in the treatment of postpartum PGP from the biomechanical point of view,so as to lay a strong theoretical foundation for the treatment of postpartum PGP by manipulation.Methods:Sixty patients with postpartum PGP were randomly divided into experimental group and control group.The experimental group was treated with pelvic plastic manipulation combined with tendon therapy,while the control group was treated with tendon therapy only.3-4 times a week,10 times a course of treatment.ASLR test,JOA score,QBPDS score and VAS score were used for scoring the efficacy before and after treatment.The statistical changes of indexes were analyzed through correlation analysis and statistical analysis by SPSS20.0 statistical software.The normal distribution of measurement data is expressed as mean + standard deviation(x±s),paired T test compared with group before and after treatment of said data;two independent samples T test indicated before and after treatment in two groups of data comparison;Spearman correlation test was used for correlation analysis,P<0.05 said there were significant differences.Results:1.Comparison of clinical efficacy: There was a significant difference in the effective rate between the experimental group and the control group(P < 0.05),and the experimental group was significantly higher than the control group.2.After treatment,JOA score,VAS visual analogue score of the patients in the experimental group and the control group;QBPDS score and ASLR score were significantly improved,the difference was statistically significant(P < 0.05);The experimental group was significantly higher than the control group(P < 0.05).3.After treatment,the difference of hip width and iliac crest height between the experimental group and the control group decreased significantly(P < 0.01),and the difference between the experimental group and the control group was significantly lower(P < 0.05).4.After treatment,the obturator width difference and lumbosacral angle of the experimental group were significantly reduced,with statistical significance(P < 0.01),and the experimental group was significantly smaller than the control group;the obturator width difference and lumbosacral angle of the control group were not improved,with no statistical significance(P > 0.05).5.The correlation analysis of QBPDS,JOA and ASLR scores before and(1)after treatment: The QBPDS,JOA score and QBPDS,ASLR score before treatment in the experimental group and the control group had significant statistical significance,and the correlation coefficient was |r| > 0.8,indicating that the QBPDS,JOA score before treatment and QBPDS,ASLR score before treatment in the two groups had significant correlation;(2)The scores of QBPDS,JOA,ASLR and the difference before and after treatment in the experimental group and the control group had significant statistical significance,but the correlation coefficients were 0.4 < |r|< 0.8,indicating that the scores of QBPDS,JOA and ASLR in the two groups were moderately correlated after treatment,and the differences of QBPDS and ASLR in the control group were low correlated before and after treatment.Conclusion:Pelvic plastic manipulation combined with massage tendon therapy has obvious advantages in the treatment of postpartum pelvic pain and pelvic functional recovery,and its clinical efficacy is better than traditional tendon massage.Pelvic plastic manipulation can restore the symmetry of pelvic ring structure and improve the overall stability of pelvic ring and realive the function of pelvic joint,improve the mechanism of mechanical load transmission,thus restoring the important role of pelvic ring in the biomechanical relationship of human body,alleviating pelvic pain in women after childbirth,allowing women patients to rebuild their self-confidence and improving their quality of life and work.Therefore,under the guidance of biomechanics theory,Bone-setting manipulation has certain reference value in the clinical treatment of traditional postpartum rehabilitation and pregnancy-related spinal diseases.
