Objective:To probe the expression of serum proteoglycan 4?PRG4?and amyloid A?SAA?in patients with Chronic Obstructive Pulmonary Disease?COPD?and its value in acute exacerbation of COPD.Methods:In this study,55 patients with acute exacerbation of chronic obstructive pulmonary disease?AECOPD?admitted to the second affiliated Hospital of Dalian Medical University from June 2018 to January 2019 were selected as the AECOPD group;22 patients with stable chronic obstructive pulmonary disease?sCOPD?were enrolled to the sCOPD group;19 cases who underwent physical examination in the physical examination center of our hospital were the healthy control group.The average age of AECOPD group was?72.75±9.97?years old,there were 40 males and15 females.The average age of sCOPD group was?73.59±7.99?years old,18 cases were male and 4 cases were female.The mean age of the healthy control group was?68.42±5.45?years,including 14 males and 5 females,there were no statistical differences between the three groups in age and gender.Smoking index and body mass index of each group were collected and lung function of each group was measured.Serum levels of PRG4,SAA and hs-CRP were detected by ELISA.Using SPSS 22.0 statistical software to analysis,in the measurement data,Using t test to compare the diffrence of datas between two groups;analysis of variance was used for the comparison among groups;chi-square test was used for counting data,when P<0.05,it is statistically significant.Result:?1?The smoking index of the three groups were?26.06±25.46?package years,?21.61±17.77?package years and?6.18±10.94?package years respectively.Comparing to the healthy control group,the smoking index of the AECOPD group and the sCOPD group was higher?P<0.05?,there was no significant difference between the AECOPD group and the sCOPD group.Body mass index?BMI?of the three groups were?25.24±4.02?kg/m2,?24.08±3.33?kg/m2 and?25.24±4.52?kg/m2respectively,there was no significant difference in the difference between the three groups of body mass index?P>0.05?.?2?The FEV1/FVC of AECOPD group,sCOPD group and healthy control group were?56.52±9.88?%,?69.98±10.80?%and?81.88±6.41?%,respectively,that of the AECOPD group was lower than the other two groups?P<0.05?;the sCOPD group was lower than the healthy control group?P<0.05?.The FEV1%pred of the three groups were?56.34±20.57?%,?56.92±20.10?%,?92.60±14.45?%,respectively,Compare to the healthy control group,the FEV1%pred of the AECOPD group and the sCOPD group were lower?P<0.05?.?3?The serum PRG4 levels in the AECOPD group,sCOPD group and healthy control group were?9.35±2.63?ng/ml,?6.52±1.05?ng/ml,?5.07±2.08?ng/ml,respectively,that of the AECOPD group was significantly higher than the sCOPD group and the healthy control group?P<0.05?;compare with the healthy control group,the PRG4levels in sCOPD group was higher?P<0.05?.The SAA of the three groups were?130.12±129.90?mg/L,?8.52±4.97?mg/L and?5.88±2.62?mg/L,respectively,the SAA in AECOPD group was higher than which in the other groups?P<0.05?,but,there was no significant difference in sCOPD group and healthy control group;The serum hs-CRP levels in the the AECOPD group,sCOPD group and healthy control group were?41.68±58.77?mg/L,?1.84±3.29?mg/L,?0.76±0.76?mg/L,respectively,the AECOPD group was significantly higher than the other two groups?P<0.05?,compare to the healthy control group,the sCOPD group was slightly higher?P>0.05?.?4?The Serum PRG4 levels in COPD patients was negatively correlated with FEV1/FVC?%?and FEV1%pred,?r=-0.547,-0.380?.The Serum SAA levels was negatively correlated with FEV1/FVC?%?and FEV1%pred,?r=-0.481,-0.292?.?5?The sensitivity of serum PRG4,SAA and hs-CRP to the diagnosis of COPD were87.0%,83.1%and 66.2%,respectively,The specificity of the indicators were 68.4%,100%and 94.7%,respectively,The sensitivity of PRG4 was higher than SAA and hs-CRP,the specificity of SAA was higher than PRG4 and hs-CRP.Conclusion:PRG4 is expressed in the serum of COPD patients and may be a potential inflammatory marker.Combined detection of PRG4 and SAA in serum is valuable for the determination of AECOPD...
