Objectives:To explore the diagnostic value and efficacy of BI-RADS classification for breast mass lesions.At the same time,the diagnostic efficacy of multiple quantitative parameters of DKI,IVIM and stretch-index model in the separate and combined differentiation of breast mass lesions was explored,and whether the three models could be used as a new non-invasive method to identify benign and malignant breast masses.Materials and Methods:1.75 cases of breast occupying disease to be diagnosed by the department of breast surgery of our hospital from September 2017 to October 2018 were collected.65 cases meeting the inclusion criteria were included in the study.There were 31 cases of benign mass lesion,including 22 cases of fibroadenoma,4 cases of inflammatory lesion,3cases of intraductal papilloma and 2 cases of cyst.2.Two groups of breast mass lesions of ADC,MK,MD,D,f,D*,DDC and alpha was statistically significant difference between the groups?P<0.05?,which the average is greater than the benign group MK malignant group,the mean parameters for benign group than the malignant group,in which the ADC,MK,MD,D,f,D*,DDC and?in differentiating benign and malignant breast lesions of the area under the ROC curve?AUC?were 0.898,0.964,0.919,0.919,0.621,0.918,0.621,0.918,The thresholds were1.44×10-3mm2/s,0.678,1.612,1.250×10-3mm2/s,2.47×10-3mm2/s,0.076,1.48×10-3mm2/s,and 0.904.The sensitivity of diagnosis was 93.8%,96.9%,90.6%,87.5%,50%,93.8%,90.6%,78.1%,respectively.The specificity was 81.2%,87.5%,87.5%,87.5%,87.5%,84.4%,81.2%and 65.6%,respectively.3.Among the combined indexes,there were statistically significant differences in the combined indexes of MK+MD,D+D*+f,DDC+?,MK+MD+D+D*+f+DDC+?between the two groups?P<0.05?.The area under the ROC curve?AUC?of the combined indexes of MK+MD,D+D*+f,DDC+?and MK+MD+D+D*+f+DDC+?in differentiating benign and malignant breast mass lesions was 0.983,0.973,0.849,0.980,and 0.996,respectively.The sensitivity of diagnosis was 96.9%,100%,93.8%,93.8%,96.9%,and the specificity was 93.7%,90.6%,78.1%,96.9%,96.9%,respectively.Rseults:1.Multi-category indexes based on BI-RADS are of reliable value in the identification of benign and malignant breast mass lesions.2.DKI parameters MK,MD,IVIM parameters D,D*,f and stretch-index parameters DDC and?have reliable differential value in the diagnosis of benign and malignant breast mass lesions,among which DKI model parameter MK value has the highest diagnostic efficiency among the three model parameters.3.Among the three models,DKI model parameters combined diagnosis of breast mass lesions has the highest efficacy,and all parameters combined diagnosis and differential value of the three models is the highest.Conclusion:Based on BI-RADS multiple classification index for the identification of benign and malignant breast lesions has reliable value.MK DKI parameters,MD,D,f,D*IVIM parameters and DDC,?tensile index parameters in diagnosis of benign and malignant breast lesions has reliable identification value,including DKI MK model parameter value in the diagnosis of the three model parameters is the highest efficiency,DKI parameter joint diagnosis efficiency is highest in three kinds of models,all three kinds of model parameters on the joint diagnosis to identify the highest value.
