Objective:To explore the value of CT combined with CA125 and HE4 in neoadjuvant chemotherapy for epithelial ovarian cancer. Methods:To retrospectivel y collect the clinical data of 61 patients with epithelial ovarian cancer confirme d by pathology after optimal primary cytoreductive surgery and treated with plat inum combined with paclitaxel in our hospital from July 2014 to June 2018.To comprehensively analyze the CT diagnosis, serum carbohydrate antigen 125(CA125),human epididymal protein 4(HE4),serum carbohydrate antigen 199(CA199)lev els before and after chemotherapy, and to evaluate the effect of chemotherapy co mprehensively. Results:CT findings before and after chemotherapy:Mean maximu m diameter of the tumor(P<0.001),ascites volume(P<0.001),peritoneal thickening(P<0.001),lymphadenopathy(P<0.001)were significant in evaluating the efficacy;t he nature of the mas(P=0.958)and the presence or absence of invasion of the s urrounding tissues(P=0.266)had no definite significance for the evaluation of ef ficacy.Tumor markers before and after chemotherapy:CA125(P<0.001)and HE4(P<0.001)levels were significant in evaluating efficacy, while CA199(P=0.253)leve ls had no definite significance for the evaluation of efficacy. There was no differ ence between CT and CRS evaluation(P>0.05).The sensitivily of CT,CA125,HE4,CT+CA125,CT+HE4,CT+CA125+HE4 in the evaluation of neoadjuvant chemoth erapy for epithelial ovarian cancer were 69.2%,66.7%,64.1%,74.4%,89.7%,92.3%,crude accuracy were 75.4%,73.8%,72.1%,78.7%,86.9%,88.5%,diagnostic index wer e 155.6%,153.0%,150.5%,160.7%,171.6%,174.1%,and Younden' index were 55.6%,53.0%,50.5%,60.7%,71.6%,74.1%.Conclusion:CT had an important value in in t he evaluation of neoadjuvant chemotherapy for epithelial ovarian cancer, measurin g mean maximum diameter of the tumor before and after chemotherapy, evaluatin g the amount of ascites, the degree of peritoneal thickening and determining lymph node enlargement are significant for evaluating the efficacy of neoadjuvant c hemotherapy for epithelial ovarian cancer, which can provide imaging basis for cl inical practice. The nature of the mass and the presence or absence of invasion of the surrounding tissues cannot provide a basis for efficacy evaluation.CT+CA125+HE4 combined examination has higher diagnostic value than single examina tion in evaluating the efficacy of neoadjuvant chemotherapy for epithelial ovaria n cancer.
