Objective: Optic neuritis(ON)refers to a variety of inflammatory lesions involving the optic nerve,often with repeated acute attacks.This type of optic nerve disease is the most common blinding eye disease among young and middle-aged people.Idiopathic demyelinating optic neuritis is similar to the clinical symptoms of MOG antibody-positive optic neuritis.It is difficult to differentiate and diagnose,and the treatment effect is poor.The choice of early correct treatment plan has improved the prognosis of patients.The pathogenesis of the disease is further explored to further elucidate the possible role and mechanism of miRNA in the pathogenesis,and to screen out specific miRNAs in cerebrospinal fluid to achieve early diagnosis,early treatment,and even to achieve therapeutic goals by interfering with and modifying miRNA.Methods: Four patients with MOG antibody-positive optic neuritis and 5patients with IDON were enrolled in the cerebrospinal fluid specimens.The miRNAs in cerebrospinal fluid samples from patients were sequenced and analyzed,and the number of small RNAs in cerebrospinal fluid samples was counted to screen for differential miRNAs and specific expressions.Results:Among the small RNAs in the cerebrospinal fluid,the measurable miRNA content was significantly higher than that of the small RNA,and the miRNAs with significant differences were screened out.41 miRNAs were found to be significantly different between the MOG and IDON groups,among which 41 miRNAs were obtained.The four miRNAs are known miRNAs,one miRNA isup-regulated,three miRNAs are down-regulated,37 unknown miRNAs,seven miRNAs are up-regulated,and 30 miRNAs are down-regulated.Conclusion:Cerebrospinal fluid is a biological sample suitable for the analysis of miRNA as a biomarker of neuroinflammatory diseases.The different miRNA distributions of IDON patients and MOG patients further support two concepts representing different diseases.Sequencing analysis of miRNAs indicates that miRNAs that differ in the two diseases are likely to be useful as diagnostic biomarkers.But it also needs to be verified in the crowd.In addition to its potential role as a biomarker,miRNAs may play a role in the pathophysiology of IDON and MOG antibody-positive optic neuritis.
