Prospective Randomized Controlled Study Of PPH To Treat Annular Mixed Hemorrhoids

Objective:To compare and evaluate the clinical efficacy and safety of Procedure for prolapseand hemor-rhoids(PPH)and traditional Milligan--Morgan in the treatment of annular mixed hemorrhoids.Methods:From December 2016 to June 2018,122 patients with annular mixed hemorrhoids who were treated in the anorectal department of the First Affiliated Hospital of Xinjiang Medical University were randomly divided into two groups:PPH group and MMH group,with 61 patients in each group.The postoperative anal pain,total operation time,intraoperative traction discomfort,postoperative anal bleeding,postoperative perianal edema,postoperative anal outlet stenosis,and total postoperative hospital stay of the two groups were prospectively compared and observed to objectively evaluate the postoperative clinical efficacy of the two groups.Result:Compared with MMH group of external dissection and internal ligation,PPH group of stapled hemorrhoid prolapse and hemorrhoids has obvious traction and discomfort feeling during the whole operation,but the postoperative anal pain is less,the operation time is significantly shorter than MMH group,the postoperative perianal edema is less obvious than MMH group,the occurrence of anal outlet stenosis and postoperative anal bleeding are less than MMH group,and the two groups of patients are different and statistically significant(P<0.05);However,after comparing the total hospitalization time between the two groups statistically,it was found that the P value was>0.05,suggesting that there was no significant difference.Conclusion:In the treatment of annular mixed hemorrhoids,stapler hemorrhoid prolapse and hemorrhoidectomy is better than thetraditional external dissection and internal ligation,and has better protection on the physiological function of anal orifice.However,the long-term effect of PPH is still unclear,and further long-term follow-up and large sample clinical trials are needed.
