Objective:To evaluate the role of adjuvant chemoradiotherapy on patients with resectable gastric cancer.Methods: A total of 223 patients with resectable gastric cancer treated at Xinjiang Medical University first affiliated Hospital from2012 to 2015 were retrospectively analyzed and were divided into the adjuvant chemoradiotherapy group(CRT;n=75)and the adjuvant chemotherapy group(CT;n=148).All patients in two groups treated with gastric resection.3-4 weeks after Patients in CRT were treated with 4-6 cycle of adjuvant chemotherapy and adjuvant chemoradiotherapy(45-50Gy/25-27f/5-6W),the CT group patients were treated with 4-6 cycle of adjuvant chemotherapy.Results:In CRT and CT group the 1,2,3-year PFS were91.5%?62.4%?54.7% and 77.7% ? 56.2% ? 33.9%(P=0.019),the1,2,3-year OS shows no statistical difference(P=0.391).Local reccurence rate were 17.3 ? 31.1%(P=0.028)and distant metastasis rate shows no statistical differenc(P=0.541)between two group.There were correlation including N state,metastasized or not,resection type,pathological type,and surgical margin with OS.And following 4 factors of metastasized or not,surgical mode,pathological type,and surgical margin are the independent prognostic factors of OS.N state,number of lymph node dissection,pathological type,margin and adjuvant treatment were the prognostic factors for PFS.And following 3factor of N state,pathological type and surgical margin are the independent prognostic factors of PFS.Among them the number of lymph node dissection and adjuvant radiotherapy was an protective factor.Conclusions:For locally advanced gastric cancer,the addition of adjuvant chemoradiotherapy after resection was associated with a significant benefit in DFS,but there was no difference in OS between two groups.adjuvant chemoradiotherapy can control local recurrence to improve patients life quality.The patients with metastasized,have total gastrectomy,signet ring cell cancer,and positive sirgical margin have a poorer prognosis.Wide range of lymphadenectomy and anjuvant radiotherapy can improve the prognosis of gastriccancer.
