Analysis Of The Influencing Factors Related To Early Complications After Hepatectomy

Objective: To evaluate the risk factors for early complications after Hepatectomy,complications with review of the literature and clinical observation to discuss the relevance of target for give advice to surgery.Methods: Retrospective analysis the clinical data of 402 patients who was undergone partial hepatectomy at the First Affiliated Hospital of Xinjiang Medical University for various benign and malignant liver diseases during Dec.2013 to Dec.2018,and divide them into postoperative complications group and non-complication group according to whether within 30 days after surgery complications.The clinical characteristics,preoperative and postoperative auxiliary examination results and surgical related factors were analyzed by single factor and multivariate(Logistic)regression to comparison.Results: Among the 402 patients,109 of them had 266 complications within 30 days after surgery,including 6 deaths.The morbidity of early postoperative complications was 27.1%,and the mortality was 1.5%.Early postoperative major complications included: Pleural Effusion 52 cases(19.5% of all complications),38 cases of Peritoneal Effusion(14.3%),24 cases of Bile Leakage(9.0%),21 cases of Subgingival Infection(7.9%),19 cases of Deep Venous Thrombosis of Lower Extremities(7.1%),17 cases of Wound Infection(6.4%),12 cases of Urinary Tract Infection(4.5%),9 cases of Respiratory Failure(3.4%),Abdominal Infection in 8 cases(3.0%),Hypohepatia in 8 cases(3.0%),Liver Failure in 10 cases(3.8%),Lung Infection in 7 cases(2.6%),Death in 6 cases(2.3%).Multivariate(Logistic)regression analysis showed that: age,preoperative cirrhosis,preoperative Child-Pugh liver function grading,duration of surgery and postoperative drainage time were associated with early complications after partial hepatectomy.Conclusions: The key to avoiding early complications after hepatectomy is to fully evaluate the liver function of the patient before operation;shorten the operating time as much as quickly;and remove the postoperative drainage tube as soon as possible.The elderly patients need to be cautious in performing liver resection.
