Objective :To observe the therapeutic value of umbilical cord mesenchymal stem cells(MSCs)on the interstitial fibrosis in advanced stage.Methods(1)Clinical section: We observed 1 case of systemic lupus erythematosus(SLE)patient who at the same time with interstitial pneumonias.We give the patient with MSCs treatment on the basis of conventional treatment and observe the clinical manifestations and the chest CT.(2)Animal experiment section: We randomly divided the 39 male kunming mice into 3 groups,normal control group(NC group)including 12 mice,bleomycin group(PC group)including 12 mice,and umbilical cord mesenchymal stem cells' treatment group(MSCs group)including 15 mice.In the 0th day BLM was injected into the trachea of the mice in the PC group and MSCs group,and the mice in the NC group were injected with the same amount of saline.After 21 days,3 mice in MSCs group were randomly selected to be dissected to observe pulmonary fibrosis,and MSCs were injected into the tail vein of the rest mice of MSCs group.To study the effect of MSCs on the advanced pulmonary fibrosis of the mice induced by bleomycin,all the mices were executed after 42 days,and the lung wet weight,hydroxyproline and the expression of transforming growth factor beta(TGF-?)and interferon-gamma(IFN-?)were compared between NCgroup with PC group and PC group with MSCs group.The experimental data were statistically processed with SPSS19.0 statistical software.Results(1)Clinical section: The patient had a severe cough and difficulty in breathing after activity and double lung has formed obvious fiber.When we give the patient with MSCs treatment on the basis of conventional treatment the symptoms and fiber cable disappeared.(2)Animal experiment section:The mice of PC and MSCs group is thin and in poor spirit and fibrosis was formed in the lung tissue of mice of the MSCs group after 21 days.There are 1 mice died in the NC group after injection of saline into the trachea 4 days later and 3 of mices died at the day of 3 and 4 and 2 of mices died at the day of 35 and 38 in the PC group after injection of BLM into the trachea and 3 of mices died at the day of 3 and 4 in the MSCs group after injection of BLM into the trachea.In the lung tissues of the MSCs group mices we observed the pulmonary alveolar texture disorders,widened alveolar interval and pulmonary fibrosis in the light microscope in 21 days in the pathological sections.After 42 days,the lung tissue alveolar texture of the mouse in the NC group was clearly visible and there was no alveolar interval widening.The alveolar texture disorder in PC group and was more difficult to identify and the spacing was significantly widened.The alveolar texture of MSCs group was clear and easy to recognize,and the alveolar interval was widened.The PC group was significantly higher than the NC group(P < 0.01)and MSCs group(P < 0.05)in in comparison of Lung wet weight,hydroxyproline content and TGF-?.The MSCs group was significantly higher than the PC group(P < 0.01)and PC group was significantly higher than the NC group(P < 0.01)in in comparison of IFN –?.Conclusion : MSCs may have therapeutic effect on interstitial pneumonia patients;MSCs in advanced lung fibrosis in mice still has a protective effect.It can effectively reduce the pulmonary fibrosisinduced by bleomycin and inhibit the expression of TGF-? and increase the expression of IFN-?in the lung tissue of mice.It showed that the MSCs to the advanced lung fibrosis still have remedial effect.
