Objectives1.To understand the status of sleep quality,cognitive function and self-management ability of CHD patients.2.To analyze the correlation between sleep quality,cognitive function and self-management ability of CHD patients.MethodsThis study was a relevance study in non experimental studies,using a questionnaire survey.From December 2017 to August 2018,220 CHD patients in the Department of Cardiology of 2 Class A hospitals in Changsha were investigated by using the target sampling method.The questionnaire included self-designed general information questionnaire,Pittsburgh Sleep Quality Index(PSQI),Montreal Cognitive Function Assessment Scale(MoCA)and Coronary Heart Disease Self-management Behavior Scale(CSMS).SPSS21.0 was used to input the data and make statistical analysis:descriptive analysis was used for general data;rank sum test and multiple linear regression analysis were used for measurement data because the analysis data did not obey normal distribution;Spearman was used for correlation analysis.Statistical test significance level was expressed as P<0.05.Results1.The PSQI score of CHD patients in this study was(5.53 ± 3.3 2).The total score of 37 patients in this study was more than 7.The incidence of sleep disorders was 16.82%,which was higher than the domestic norm(3.88±2.52).The scores of subjective sleep quality,sleep delay and sleep disorder were in the top three of the seven dimensions measured by PSQI,with the average scores of(1.30±0.59),(1.11±0.98),(1.29±0.54),respectively.2.The MoCA score of CHD patients in this study was(24.38+8.41).Among 220 subjects,104 of them scored less than 26,and the incidence of cognitive impairment was 47.27%.The scores of 7 dimensions measured by MOCA were(3.61+1.75)in visual space and executive function,(2.67±0.62)in nomenclature,(5.19± 1.07)in attention,(1.86±1.02)in language,(1.44±0.78)in abstraction,(3.35±1.67)in delayed recall,and(5.66±1.01)in orientation.3.In this study,the CSMS score of CHD patients was(82.93 ±15.10),32 patients with CSMS score>100 accounted for 14.55%;100 patients with 80-99 score accounted for 45.45%;72 patients with 60-79 score accounted for 32.73%;16 patients with 46-59 score accounted for 7.3%.The first five dimensions of CSMS scores were:emotional cognitive management(1.77±5.30),bad habits management(15.49±2.79),daily life management(12.94±3.04),symptom management(9.70±3.79),emergency management(8.79 ± 3.84).4.the sleep quality of CHD patients was positively correlated with cognitive function.The correlation between PSQI total score and MoCA total score was statistically significant,with a negative correlation coefficient of 0.112;the correlation between PSQI total score and MoCA dimension abstraction and orientation was statistically significant,with a negative correlation coefficient of 0.165 and 0.325;the correlation between PSQI sleep disorder,daytime function and MoCA total score was statistically significant,with a negative correlation.The coefficients are 0.334 and 0.220 respectively.The correlation between subjective sleeping time and orientation dimension was statistically significant,with a negative correlation coefficient of 0.152;the correlation between sleep time and visual space and executive function,abstraction and orientation dimension was statistically significant,with a correlation coefficient of 0.156,0.237,0.272;the correlation between sleep efficiency and ion and delay was statistically significant.The correlation of memory was statistically significant,with a negative correlation coefficient of 0.205,0.149;the correlation between dimension sleep disorder and visual space and executive function,nomenclature,memory,attention,abstraction,delayed memory and orientation was statistically significant,with a negative correlation coefficient of 0.243,0.244,0.343,0.288,0.211,0.298,0.443,respectively.The correlation coefficients of daytime delay and visual space with executive function,nomenclature,memory,attention,language,delayed memory,orientation and total cognitive score were 0.137,0.240 and 0.227 respectively.0.236,0.211,0.194,0.180,0.201,0.327,0.220.5.The correlation between sleep quality and self-management ability of CHD patients was statistically significant,and the correlation was positively correlated.The correlation between the total score of PSQI and the total score of CSMS was statistically significant,with a negative correlation coefficient of 0.142.The correlation between subjective sleeping time and disease knowledge management,daily life management and total score of self-management was statistically significant,with a negative correlation coefficient of 0.184,0.205,0.139,and a positive correlation of 0.139,0.244.Dimensional sleep efficiency was positively correlated with Emotional Cognitive Management,Emergency Management,Disease Knowledge Management,and Self-management Scale,with a correlation coefficient of 0.345.Dimensional sleep drugs were positively correlated with the total scores of Emotional Cognitive Management,Emergency Management,Disease Knowledge Management and Self-management Scale,with a correlation coefficient of 0.139,0.185,0.184,0.208.The correlation between the total score of the diurnal function and the dimension of disease knowledge management,daily life management and self-management scale was negative,with a coefficient of 0.139,0.305,0.170;the correlation between the total score of sleep and daily life management was statistically significant,with a coefficient of 0.234.Conclusions1.The sleep quality of CHD patients is poor,16.82%of CHD patients have sleep disorders,PSQI scores higher than the domestic norm.The cognitive function of2.CHD patients was poor,and the MoCA score was(24.38 ± 8.41)points,which was lower than the MoCA cut-off value of 26 points.3.CHD patients' self-management ability is poor,CSMS score is(82.96+15.10)points,accounting for 61.42%of the total score.4.There is a positive correlation between sleep quality and cognitive function and self-management ability in CHD patients,that is,the better sleep quality,the better cognitive function of CHD patients,and vice versa;the better sleep quality,the higher self-management ability of CHD patients,and vice versa.
