Platelet Count To Lymphocyte Count Ratio May Predict Mortality In Type B Acute Aortic Dissection

Background: Acute aortic dissection(AAD)is a common disease with a high death rate.Inflammation activated by the tearing of the aortic wall is the pathological basis of AAD.Many factors influence the extent and prognosis of AAD;particularly platelets are inextricably bound with AAD.This study aimed to identify the relationship between the platelet-to-lymphocyte ratio(PLR)and the mortality of type B AAD.Methods: From2007 to 2012,134 patients with type B aortic dissection included in the study,and were divided into three groups.Race,gender,age,past medical history,white cell,platelet count,lymphocyte count,complications,treatment and the drugs and so on,were measured in all patients.Cox proportional hazards regression,Kaplan-Meier curves and logistic analysis were performed.The relationship platelet count to lymphocyte count ratio and mortality was assessed in type B acute aortic dissection.Results:According to the best cut-off value,134 patients were divided into high PLR group and low PLR group.The survival time was lower in the high-PLR group than in the low-PLR group(13.54±13.85 vs 33.87±19.18).Compared with the low-PLR group,the high PLR group experienced many complications,and the percentage of in-hospital survival and the survival at the end of follow-up was more.Correlation analyses and Cox regressions found that the following variables were independently associated with mortality: Smoking(OR,7.74;95% CI,1.261-47.564;P=0.03),complications(OR,15.08;95%CI,2.444-93.081;P=0.000),and PLR(OR,6.14;95%CI,1.401-26.895;P=0.02).These results indicate that the PLR is a risk factor for type B AAD.Conclusions: We found that the PLR was an independent risk factor for type B AAD.The PLR was higher in the death group,and patients with a high PLR had a higher risk of developing complications,which increased the mortality of type B AAD.We conclude that the PLR could be used to evaluate the prognosis and assist physicians in selecting treatments.
