Study On The Cognition Of Family Doctor Contract System Between Doctors And Patients In Xiamen

The "three divisions co-management" family doctor signing service model in Xiamen is one of the five family doctor signing models promoted by the National Health and Health Commission,but there is no systematic discussion about the perceptions of both doctors and patients.At present,most of the cognition research on family doctor signing system focuses on one of them,especially the cognition of the resident,but there are few comparative studies on the doctors and patients.This study is based on the implementation of the Xiamen family doctor signing system for nearly one and a half years to conduct a practical questionnaire survey,comprehensively expounding the comparison of doctors and patients based on contract services on the family doctors signing system,in order to improve the family doctors signing rate Suggestions.The empirical data show that the doctors and patients in Xiamen have slightly different cognitions about the connotation and meaning of the system,and the cognition of the family doctor signing system is more consistent.The concept of family doctors' signing system was 80.88%and 78.77%,respectively;the support rate for the system was 61.61%and 45.0%,respectively.In practice,the signing rate of the family doctors of residents is 43.88%,and the rate of first-time consultation at the grassroots level is 63%,which needs to be improved;The enthusiasm of medical staff to participate in family doctors is high.51.5%of medical staff signed more than 151 residents in 2017,and 60.75%of medical staff are eager to participate in family doctors' contract reform and service.Therefore,according to the cognitive-behavioral theory and the theory of"knowledge,belief and practice",the author suggests that we should widen the channels to increase publicity,raise the awareness rate of both doctors and patients on the contract-signing system of family doctors,improve the belief in internalization of demand,improve the support rate of both doctors and patients on the contract-signing system of family doctors,establish an effective contract-restricting mechanism,and enhance the contract between doctors and patients.About the rate of performance,improve the satisfaction of doctors and patients,and promote the signing of a family doctor system into a virtuous circle.At the same time,this study investigates the cognition of both doctors and patients to the system of family doctor's contract signing,and finds out the influencing factors,which has certain research significance to promote the better implementation of family doctor's contract signing behavior.
