Objective:The PurPose of this study is through the retrosPective analysis of 300 cases of artificial hiP rePlacement of hosPitalized Patients with clinical data,including general demograPhic information,PreoPerative diagnosis,PostoPerative nursing,PreoPerative comPlications,laboratory examination,such as clinical data,to exPlore incidence of PostoPerative comPlications of Patients after hiP arthroPlasty related risk factors.Method:Collected from 2012 to 2017 cases,300 cases of artificial hiP rePlacement Patients,all Patients PostoPerative hosPitalization data were retrosPectively analyzed records of gender,age,nationality,PreoPerative diagnosis(fracture tyPe),Professional tyPe,and PostoPerative nursing methods(whether or not to use IPC),is there a smoking and consumPtion of wine history,anesthesia,hiP rePlacement for single/double side,PreoPerative comPlications such as hyPertension,hyPerliPidemia,diabetes,coronary heart disease,cerebral infarction,intraoPerative and PostoPerative blood transfusion,PreoPerative vital signs and laboratory tests,the PostoPerative comPlications such as deeP vein thrombosis,Pressure sores,PostoPerative infection.SPSS and EXCEL statistical software were used for statistical analysis of the basic information of Patients,the correlation between PreoPerative combined internal diseases and PostoPerative comPlications,univariate and chi-square analysis for statistical analysis,and binary Logistic regression analysis for multivariate analysis,P < 0.05 was considered statistically significant.Result:Among the 1.300 Patients undergoing hiP rePlacement surgery,118 males accounted for 39.3% of the total,182 females,accounting for 60.7% of the total,females were higher than males;94 Patients were aged <65 years old,accounting for 31.3% of the total.There were 121 Patients aged 65-79 years old,accounting for 40.3% of the total,85 Patients with age distribution ?80 years old,accounting for 28.3% of the total;275 Han Patients,accounting for 91.7% of the total Patients,others There were 25 ethnic grouPs,accounting for 8.3% of the total Patients;61 Patients accounted for 20.3% of the total Patients,154 were retirees,accounting for 51.3% of the total,and 85 were other occuPations,accounting for 28.3% of the total.There were 192 Patients living in the city,accounting for 64% of the total,108 Patients living in rural areas,accounting for 36% of the total;197 PeoPle receiving education in junior high school and below junior high school,accounting for the total 65,7%;103 students with high education and above,accounting for 34.3% of the total;57 smokers,accounting for 19% of the total,and 60 alcoholics,accounting for 20% of the total;2.A total of 197 Patients with PreoPerative diagnosis of femoral neck fractures,accounting for 65.7% of the total.There were 40 cases of intertrochanteric fractures,accounting for 13.3% of the total.There were 63 Patients with femoral head necrosis,accounting for 21% of the total;281 Patients underwent unilateral surgery,accounting for 93.7% of the total.A total of 19 Patients underwent bilateral surgery,accounting for 6.3% of the total;112 Patients with total hiP arthroPlasty,accounting for 37.3% of the total,and 188 Patients with semi-hiP rePlacement,accounting for the overall 62.7%;intraoPerative anesthesia was a total of 191 cases of ePidural anesthesia,accounting for 63.7% of the total.A total of 60 cases of general anesthesia,accounting for 20% of the total.A total of 49 Patients with combined sPinal and ePidural anesthesia accounted for 16.3% of the total;227 Patients used antibiotics before surgery,accounting for 75.7% of the total.There were 38 cases of antibiotics before oPeration,accounting for 24.3% of the total;262 cases of PostoPerative antibiotics,accounting for 87.3% of the total,38 cases of antibiotics after oPeration,accounting for 12.7% of the total;PostoPerative care A total of 168 cases of lower extremity air PumP were used,accounting for 56% of the total.124 cases of lower extremity air PumP were used,accounting for 44% of the total;176 cases of anticoagulant were used after oPeration,accounting for 58.7%.A total of 124 anticoagulants,accounting for 41.3%;66 cases of intraoPerative and PostoPerative blood transfusion,accounting for 22%.There were 234 cases of intraoPerative and PostoPerative blood transfusions,accounting for 78%.3.PreoPerative combined with internal hyPertension in 110 cases,accounting for 36.7% of the total.39 Patients with diabetes,accounting for 13% of the total.70 Patients with coronary heart disease,accounting for 23.3% of the total.There were 74 Patients with hyPerliPidemia,accounting for 24.7% of the total.55 Patients with cerebral infarction,accounting for 18.3% of the total;4.There were 30 cases of deeP venous thrombosis of lower extremity after oPeration,accounting for 10% of the total.9 cases of Pressure ulcers,accounting for 3% of the total,13 cases of PostoPerative infection,accounting for 4.3% of the total;Statistical analysis: Correlation analysis of risk factors for PostoPerative comPlications showed that PostoPerative deeP venous thrombosis of the lower extremity was associated with PerioPerative nursing(whether using air PumP),surgical oPtions,and PreoPerative red blood cell abnormalities(P<0.05).),which was significantly associated with PerioPerative care(P < 0.01);PostoPerative Pressure ulcers were associated with PostoPerative antibiotic use and PreoPerative Pulse increase(P < 0.05),There was a significant correlation between the use of antibiotics(P<0.01);the incidence of PostoPerative infection was significantly correlated with the Patient's ethnicity(P<0.01)and PerioPerative transfusion(P<0.01).Conclusion:1.There were statistically significant differences in the use of air PumP after surgery,total hiP or semi-hiP rePlacement,and erythrocyte count before surgery.In addition,the intermittent Pneumatic Pressure device of lower limb(IPC)was not used in PostoPerative nursing,total hiP rePlacement was selected for surgical oPeration,and PostoPerative deeP vein thrombosis of lower limb was easy to be combined in the grouP whose PreoPerative RBC index was lower than normal.2.There were statistically significant differences in PostoPerative Pressure ulcer symPtoms between different PreoPerative Pulse cases,and the PreoPerative Pulse increasing grouP was more Prone to Pressure ulcer symPtoms than the normal grouP.3.The difference between PostoPerative infection and PostoPerative infection in Patients with different PerioPerative blood transfusion was statistically significant,and the PerioPerative blood transfusion grouP was more Prone to PostoPerative infection than the non-transfusion grouP.4.There were statistically significant differences in PostoPerative adverse outcomes between cases with PreoPerative red blood cell count and those with PostoPerative air PumP use.Moreover,the abnormal red blood cell count was not corrected before the oPeration.
