| BackgroundOsteoarthritis(OA)is a heterogeneous group of diseases causes’ joint abnormality which are associated with pain and disability of articular cartilage in addition to related changes in the underlying bone at the joint margin and it is the most common form of arthritis.It’s characterized by pain,tenderness,morning stiffness,deformities,and muscle weakness of the soft tissue,restricted movement,bony swelling and so on.OA can be diagnosed by physical examination,and we can use some parameters to diagnose such as CT scan,MRI and arthroscopy is also important to diagnose OA,Management of osteoarthritis are classified in to three main groups;pharmacologic,non pharmacologic and surgical intervention.Total knee arthroplasty is one of the most common procedures performed for people with severe joint problems and also prosthesis are different and depends on the underlying cause and the patient’s age.These kinds of operations are expansive,however the prevalence of OA will raise in the near future as the people ages Previous researches propose that TKA improves patients quality of life also reduces pain(36-41)on the other hand anterior knee pain(AKP)is present in 4 to 40 percent of all cases Objective of the studyThe aim of this study is to compare mobile bearing and fixed bearing TKA postoperative recovery and short time clinical outcomes using preoperative KSS,post operative KSS,pre operative HSS,post operative HSS,blood loss,pain vas as measurement.MethodsClinical data and records of 20 patients with OA of the knee underwent total knee arthroscopy TKA in the first hospital of Jilin university between January and December year 2017 I reviewed retrospectively and collected for analysis.I divide in to two different groups’ group one mobile bearing and group two fixed bearing prosthesis Based on these groups will be able to get the result from the record the surgical outcome of these groups will be noted.An Ethical approval will be obtained from the Ethics Committee of the first hospital of Jilin University before commencement of the study.Data will be analyzed using SPSS version 22.Outcome measures:The aim of this study is to compare mobile bearing and fixed bearing TKA postoperative recovery and short time clinical outcomes using preoperative KSS,post operative KSS,pre operative HSS,post operative HSS,blood loss,pain vas as measurement.Resultsresults of the fixed-bearing knee prosthesis group and the mobile-bearing knee prosthesis group(Table 2)showed: There was no statistically significant difference between age(p=0.199),gender(p=1.000),postoperative KSS(p=0.283),preoperative HSS(p=0.535),postoperative HSS(p=0.254),and the pain VAS(p=0.707)in the two groups.There was a statistically significant difference between the two groups of preoperative KSS(p=0.010<0.05).Patients in the mobile-bearing knee prosthesis group had higher preoperative KSS scores.There was a statistically significant difference between the two groups of patients with increased KSS scores(p=0.001<0.05).The KSS scores were more improved in the fixed-bearing knee prosthesis group.There was a statistically significant difference between the two groups of patients with intraoperative blood loss(p=0.037<0.05).The patients in the mobile-bearing knee prosthesis group had more blood loss during operation.ConclusionsThe two kinds of prosthesis have positive effect on the pain and quality of life of patients with severe osteoarthritis,although it is found that there is a lot of blood loss in the knee prosthesis of the mobile platform,but there is no difference in the short-term curative effect.Although theoretically mobile platform prosthesis has obvious advantages over fixed platform prosthesis in improving patients ’ activity function and reducing gasket wear,The long-term clinical effect remains to be further observed... |
